PLEASE READ AND AGREE TO THE FOLLOWING WAIVER AND POLICIES. REGISTRATION IS INCOMPLETE WITHOUT AGREEMENT AND MUST BE COMPLETED BEFORE FIRST CLASS.
LIABILITY WAIVER AND ACKNOWLEDGMENT OF RISK:
IN CONSIDERATION OF CHRISTINE’S CENTER STAGE DANCE STUDIO, LLC (“Studio”) providing dance instruction to my child or children, the undersigned understand and agree that in participating in any dance class, workshop, rehearsal or performance, in-studio or online, there is a possibility of physical injury or death. The undersigned voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Studio’s classes, rehearsals, performances, or activities. The undersigned also agree to release, hold harmless and indemnify Studio, its member, agents, volunteers, assistants, employees, staff and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Studio. The undersigned further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Studio, its member, agents, volunteers, assistants, employees, staff, and/or students liable for such damage, loss, injury, or death. If the undersigned is signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.
ASSUMPTION OF RISK AND WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19:
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person. As a result, national, state and local governments and health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
CHRISTINE’S CENTER STAGE DANCE STUDIO,LLC known herein as “the Studio” has put in place preventative measure to reduce the spread of COVID-19; however the Studio cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Studio could increase your risk and your child(ren)’s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Studio and that such exposure or infection may result in personal injury, illness, personal disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Studio may result from the actions, omissions, or negligence of myself or others, including but not limited to, Studio employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including but not limited to personal injury, disability and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’ attendance at the Studio or participation in Studio programming (“Claims“).
On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge and hold harmless the Studio, it employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Studio, its employees, agents, and representatives whether a COVID-19 infection occurs before, during, or after participation in any Studio classes, events, or otherwise.
I also agree in the case of an outbreak that my information may be shared with relevant bodies for contact tracing in the event of a child of this Studio becoming ill with COVID-19.
EMERGENCY MEDICAL ATTENTION:
Permission is granted Studio to provide emergency medical attention if necessary.
INSURANCE:
Studio does not carry medical insurance on the dancers. If injury occurs it is understood that any financial obligations for such injury are the responsibility of the dancer’s family.
STUDIO CLOSURE:
In the event that the Studio would need to close due COVID-19 or other unforeseen circumstances, classes will continue online for the duration of the closure at the same tuition rate. In person classes will resume as soon as it is deemed safe.
CLASS ATTENDANCE:
Excessive class absences may result in removal from performances (including the spring recital).
TUITION PAYMENTS:
To facilitate tuition payments and shoe, dancewear, and merchandise ordering, we are requesting that each family have a credit/debit card number kept on file or enroll in our new Auto Pay feature. Tuition may be paid by the following methods: Cash, Check, Money Order or Credit/Debit Card. Credit/debit card and ACH payments can be made online through your Parent Portal, with our Auto Pay feature, or over the phone. Cash payments must be in an envelope with the dancer’s name. Checks should be made payable to Center Stage Dance Studio and include the dancer’s full name on the memo line. Cash, checks or Money Orders may be placed in the secure drop box in the waiting room or checks may be mailed to: 624 Elm Street, Mauston, WI 53948. There will be a $35 charge on all returned checks. Full or partial payments may be made.
Account review dates are as follows:
October 1st, 2023 – one quarter of total balance must be paid
December 1st, 2023 – one half of total balance must be paid
February 1st, 2024 – three quarters of total balance must be paid
April 1st, 2024 – total balance must be paid
Accounts will be reviewed on the aforementioned dates and any account not current as stated above will be subject to a $10 late fee and the card on file will be charged or, if there is no card on file, dancer(s) will be asked to sit out of class until tuition is brought up to date.
Full account payment must be received by April 15th in order to participate in the spring recital.
SPRING RECITAL COSTUMES:
A $50 per class recital costume deposit is considered a class registration deposit and is required to hold a dancer’s place in class. Costume balances will be due Nov.1st. If a costume is not fully paid for, it will not be ordered. Costume deposit/registration deposit is nonrefundable.
SHOE, DANCEWEAR, AND MERCHANDISE ORDERS:
All shoes, dancewear, and merchandise such as water bottles and t shirts must be paid for in full at the time of purchase/order. Card on file can be used.
WITHDRAWAL POLICY:
If a class is dropped by a dancer, the Studio must be notified in writing and dancer shall be considered enrolled until such written notification is received by the Studio. Payments shall be made up to the date of receipt of such written notification.
STUDENT PHOTOS/VIDEOS:
Permission is granted Studio to use photographs and video of students for publicity purposes and social media.
PRIVACY:
All information (i.e. birthdates, contact information, credit card information, etc.) given to the Studio is kept strictly confidential. Class schedules and other information will be provided only to the registering adult.
FORCE MAJEURE:
We assume no responsibility for any personal injury, property damage or other loss, accident, delay, inconvenience, irregularity, or cancellation of all or any portion of the dance year which may be occasioned by reason of any matter beyond our exclusive control including but not limited to acts of God, acts of government, war, terrorist acts, riots, disaster, weather extremes or pandemics.
