I have read and reviewed Premier Dance Academy's policies

I am permitting the child I have registered to attend classes at Premier Dance Academy Inc. I have received or will obtain a copy of Premier Dance Academy policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Premier Dance Academy does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc.

I acknowledge that there is a risk involved in any activity for which I am registering myself and/or my child(ren). I hereby accept the risk and in consideration Premier Dance Academy Inc. accepting this registration. I hereby release Premier Dance Academy Inc. from all claims of any kind that may arise. I will allow Premier Dance Academy to use photographs/images of myself and/or my child(ren) as a participant of Premier Dance Academy Inc. on their website and/or in promotional material.

The above information is collected for the sole use of Premier Dance Academy Inc. and will not be disseminated in any way to other parties without the prior written permission of the participant/participant’s parent/ guardian.

All dancers under the age of 4 must have a Parent/ Guardian in the building with them at all times as Premier Dance Academy is not a licensed daycare.

Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

Premier Dance Academy Monthly Instalment payment plan is based on a 10 month dance year. This ensures each month is the same price whether the month is longer or shorter than the previous. All payments are due on the 1st of the month, preferably made through our Automatic withdrawal program or Autopay. We also accept e-transfer, debit/credit, cash and cheques.

Payments will only be prorated when a dancer starts later in the year.

I acknowledge that photos may be taken inside the studio. I will allow Premier Dance Academy to use photographs/images of myself and/or my child(ren) as a participant of Premier Dance Academy Inc. on their website and/or in promotional material.
Photos and videos will also be taken at recitals and performances.
If you would prefer for your child to not be in photos used for advertising simply send us an email and we will make a note on your file :)
