**Please note that any child with any signs of illness will not be allowed to participate in classes. We will follow CDC guidelines as well as Kentucky guidelines.**
The following procedures will be in place:
1. Drop off your dancer at the door. At this time, parents will not be allowed in the studio.
2. Temperature will be checked when entering the studio.
3. Dancers will be required to wash their hands before class.
4. There will be no loitering in the waiting room, once hands are washed, the dancer should go directly to the classroom.
5. Water fountains will not be available, each dancer must bring a water bottle to use.
6. Dancers will remain in the same room throughout class and be asked to maintain a 6 ft. distance between students. There are 6 ft. taped areas for dancers in the classrooms.
7. Class sizes will be limited to 10 and under.
8. Classrooms/bathrooms will be sanitized between classes.
9. Dancers should bring their dance shoes in a ziplock bag with their name on it. No dance bags will be allowed in the academy.
10. Class times will be staggered with time between classes for traffic flow and sanitation.
11. Instructors and staff will be required to wear a mask during class.
12. Dancers will not be required to wear a mask in class unless mandated by Gov. Beshear (Parents will be updated as information is released)

ACADEMY OF DANCE

________ A non-refundable $25 Registration Fee per child is due upon registration.
________Tuition is due on the 1st of each month and a 20% late fee will be charged if not paid by the 10th of each month.
Autopay is recommended and can be scheduled to run on the 1st of the month. Tuition remains the same
regardless of the number of weeks in each month. Semester and yearly payments are NON-REFUNDABLE.
________ A $35 return check fee will be added on all insufficient fund checks.
________ There are NO REFUNDS for missed classes. Students may make-up a missed class with another class of a similar
style/level at any given time when applicable and when space is available with Covid-19 restrictions.
________ I understand that there are Costume Fees in addition to my child’s tuition that will be due.
Costume fees are divided into 3 payment plans of $30 due by October 15th, $30 due by November 15th, and the remaining balance due by January 8th. All Costume fees are non-refundable.
________ All accounts must be completely paid in full by May 21, 2021 for students to be able to
participate in the Spring Recital.
________ By initialing here, I understand that any injury that is prompted from activity taking place in
the studio is, in no way, the responsibility of Beverly Rogers Academy of Dance or its owner.
________ I understand that our 2020-2021 Dress Rehearsal and Recital Dates are tentatively scheduled for
________ I understand my child must dress appropriately when attending class. This includes wearing
the correct form fitting dancewear, tights, and shoes per class. Hair must be kept pulled back
in either a pony or bun depending on class requirements.
________ I understand the Paducah studio follows the Paducah City Schools for inclement weather
closings and the Princeton studio follows Caldwell County Schools.
________ PHOTO RELEASE: By initialing here, I give permission for photographs or videos of my child in
dance class or performances to be used in promotional material for Beverly Rogers Academy
of Dance in both print and web publications.
________ I understand that if the studio is required to close due to state regulations, NO REFUNDS will be
issued. Classes will resume on zoom or be made up at a later date.
________ I understand that the studio will not be held liable for anyone who may become ill from Covid-19 or
from Covid related illnesses. We will notify you if your child has come in contact with anyone
who tests positive for Covid-19.
By signing below, you are acknowledging that you have read and understood all policies & procedures, tuition policy, and fees associated with training/performing with Beverly Rogers Academy of Dance.

I, the undersigned parent or legal guardian of the dancer listed above, do hereby give permission for the aforementioned person to participate in any and all classes, programs, shows, and events offered by or attended by Beverly Rogers Academy. I accept all risks associated with participation and understand that there is a full responsibility of serious physical illness or injury. I hereby covenant not to sue and waiver, release, and forever discharge any and all rights and claims for damages, which may arise now or in the future against Beverly Rogers Academy and its officers, owners, directors, employees, and/or other assigned representatives or volunteers from any and all liability, damages, and/or injuries which may be sustained or suffered by the dancer listed above while participating at or for Beverly Rogers Academy. I assume all risks and hazards incidental to such participation including transportation to and from all activities and so herby waive, release, absolve, indemnify and agree to hold harmless the Beverly Rogers Academy, organizers, sponsors, participants, and persons transporting students or participants to and from activities for any claims arising out of injury to the student. Furthermore, I hereby give permission to Beverly Rogers Academy to use photographs and/or video of the dancer listed above as deemed appropriate for the promotion of Beverly Rogers Academy.
