I, parent/guardian of above-mentioned student, do hereby give my consent to the administration of Radiance Ballet or their duly authorized representation to secure medical or surgical aid as may be deemed necessary and expedient in case of an emergency when parents (guardian) can not be reached. Consent is also given to the administration Radiance Ballet or their duly authorized representation to secure/provide transportation for said child to an emergency medical facility in the event that parents (guardian) can not be reached. We hereby release Radiance Ballet, its agents, and volunteer assistants from any liability whatsoever arising out of any injury, damage, or loss which may be sustained by the above-named student while participating in classes and activities offered by Radiance Ballet.

I have read and understand the dress code and policies of Radiance Ballet. I agree to abide by and support these policies to the best of my ability during my child’s participation in the program. Radiance Ballet reserves the right to change or add to the policies listed, but parents will be notified in such an instance.

When enrolling in ballet classes at Radiance, you are committing to a semester of instruction. Should you need to drop a class, you must submit a written notice to the director and pay for the remaining month of classes, despite whether or not you will be able to attend classes. By accepting this waiver, you are agreeing to pay for all classes, despite attendance history, from the registration date UNTIL the end of the month in which you give written notice of dropping the class(es) in which you or your child is enrolled.
