I understand that my child's photo/video may appear on studio website or in other advertisements to promote the studio. I consent to the release of my child's photo/video for advertising purposes. (Celebrity Dance Emporium never releases students names with photos).

We will hold a students place in class for 10 days. If registration fee is not received, made payable to Celebrity Dance Emporium, then we will drop said student from the roster. Registration fee is NON-REFUNDABLE. Costume payments are due by November 1st. No refund will be given to those students who drop out after the costume orders have been placed. Tuition is due by the first of each month, September-May. No refund will be given for students that drop out in the middle of a pay period. If accounts are paid after the tenth of the month, there will be a $10.00 late fee applied to the account balance. There is a 65$/85$ Recital fee that is due March 1st. This covers rental of the auditorium, insurance coverage for the auditorium, a digital copy of the recital, recital t-shirt and program. There is a $25.00 returned check charge for any checks returned by the bank. No refunds are given to those students that miss class for any reason. In the event of weather, pandemic or other extenuating circumstances, Distance learning will be a compensatory service to in-person classes.

As the legal parent or guardian, I understand that there are certain risks involved with participation in any performing arts activity. I release and hold harmless Celebrity Dance Emporium, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, illness or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Celebrity Dance Emporium, its owners and operators or in route to or from any of said premises.
