3D Dance Academy commits to anti bullying. We provide a safe and supportive community that fosters a lifelong love for dance and creative expression. In order to guarantee that all of our dancers are able to best pursue their passion, we implemented a comprehensive anti-bullying policy applicable to our students, dance families, staff, and employees.
We define bullying as any behavior; physical, verbal, written (either electronically or in person), that detrimentally targets another dancer. This counts as bullying when it physically or emotionally harms a dancer, targets a dancer for any actual or perceived characteristic, or creates a hostile environment in the dance studio.
We understand that the under reporting of bullying is the biggest hindrance to stopping further harassment. We have developed an online form for students or parents to file in the case of bullying.
We send this information directly to the leadership team, who are in charge of the implementation and studio wide compliance to the anti-bullying policy. The Executive team will contact the alleged bullying party, the alleged victim, teachers, assistants, and parents who may have possibly witnessed the event. Should, in addition, he/she find evidence that corroborates the report; this will serve as the basis for scheduling a course of action meeting. 3D Dance Academy Director, Kristen Robinson, will also review any report of bullying. 3D Dance Academy staff members including educators, associates, and administrative personnel must intervene if they are made aware of the possibility that bullying is occurring including but not limited to witnessing bullying. 3D Dance Academy personnel have to intervene by filing the online report directly. Parents making the studio aware of the bullying event, the parent is also required to take on the primary responsibility of filing a report.
3D Dance Academy will not tolerate bullying in any form; physical, emotional, social or otherwise. As such, we have implemented an Anti-Bullying Policy. Since our goal is to empower people and embrace individuality, one documented bullying event will have repercussions, which may include immediate termination of enrollment for repulsive violations.
Most noteworthy, as per the policy above, 3D Dance Academy recognizes that bullying does not just occur between students. Parents or guardians, who harass another student, parent, or 3D Dance Academy staff member, will have repercussions, which may include suspension or termination of student. Please report all incidents of bullying you witness to the Director immediately. There will be no refunds for such actions.
At 3D Dance Academy, respect and compassion are two of our core values. All dancers have a right to learn and develop their art in a positive, respectful environment. We realize that every situation reported through this form may require a unique follow-up.

 In the event that we should encounter another pandemic, Parents will be responsible for paying ½ of their assigned tuition. Dancers will be assigned a google classroom the first week of classes which is where they will meet for classes in the event, we are unable to meet in person. Virtual classes will take place in Google Meets and You Tube. It will be the responsibility of the dancer’s parent to ensure that dancers are continuing classes. All payments will remain as is. In the event that a recital is cancelled, dancers will still be responsible for purchasing uniforms and paying all fees. This is to ensure that when we are able to resume, we can do so and continue with our projected dates. The shipment of uniforms can be a lengthy process, please keep this in mind when ordering uniforms for recital.
 Parents are not allowed inside of the studio unless told to come in. This is to ensure the safety of dancers and staff. Temperatures will be checked before entering the building. Dancers should line up at the door and not enter without being scanned.
 If your child is sick, PLEASE KEEP THEM AT HOME!!
 IF YOU ARE SICK, A SIBLING IS SICK, OR A RELATIVE IS SICK, and the child has had any interaction with them, they SHOULD NOT ATTEND CLASS.
 Dancers must sanitize upon entering the building.

You have reserved your child's class time from the date of signature until the end of the dance year(May). You will be billed and are liable for the payment of tuition for those classes whether or not your child attends the class. If a student stops attending class without notice, and without permission to be released from the contract, all tuition and fees will be owed and collected. This is a one-year commitment contract, and you agree to pay tuition through May whether your child attends each month or not. Certain exceptions may be made due to medical illness, job change/relocation. All tuition will be due if a student drop’s out.

I hereby affirm that I am voluntarily starting a course of instruction at 3D Dance Academy (The Activity). I am voluntarily participating in the Activity entirely at my own risk. In full consideration of the risk of injury while participating in the Activity, and for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating in the Activity, which may include, but are not limited to, physical or physiological injury, pain, suffering, illness disfigurement, temporary or permanent disability, economic or emotional loss, and death I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.

I hereby grant permission for my child to participate in any, promotional purposes relating to 3D Dance Academy whether public or community, and I support and hereby grant permission to 3D Dance Academy to use my name, child’s name, to take and publish photographs, videotapes or motion pictures of my child or which include my child’s voice, in any media for any legitimate purpose. The following times may include for photography or videography will include, but are not limited to, newsletter pictures, slideshows, program activities, special events, advertising, public relations, program news articles, as well as times deemed necessary by the staff. I release all rights to such photographs, videotapes, motion pictures and recordings. I acknowledge that 3D Dance Academy will be the sole owner of all rights arising out of their use for any purpose. I understand that I shall receive no compensation from their use from any source whatsoever.

 Tuition is due the 1st- the 5th of every month. If the 5th falls on a weekend, you are responsible for ensuring the payment is made by 8pm on that day. Late fees will begin at 8:01pm. The late fee is $30 PER DANCER, and It must be paid at the time you submit the tuition. If the late fee is not made at that time, your account will be considered delinquent and the dancer will not be able to attend until all payments are caught up.
 Students with delinquent accounts will not be admitted to classes, accepted for a new year, perform for recital, perform for competitions, or perform for parades or any other event, or receive recital costumes until the account is current.

Electronic media has become a normal method of communication in the 21 st century. Social networks, texting, blogs, and all other forms of electronic communication pose certain risks to students and as such, should be used with care and maturity. It is in the best interest of 3D Dance Academy to communicate certain expectations to dance team members concerning these methods of communication. It is stated in the code of conduct rules and regulations for 3D Dance Academy that any conduct unbecoming of a member will result in disciplinary action. I fully understand that this applies not only to my conduct at school, but also in the public and that the Internet and all electronic media communication is an extension of the public. Any posts (words, photographs, graphics, etc.) by a 3D Dance Academy dance team member via any form of electronic media determined by the director and/or leadership team to be inappropriate and unbecoming of a member will result in disciplinary action with possible dismissal. Members specifically agree not to submit, publish, or display any defamatory, inaccurate, abusive, obscene, profane, sexually oriented, threatening, racially offensive, or illegal material; nor shall members encourage the use of controlled substances (i.e. alcohol, tobacco, drugs, etc.). Violations will generally be treated in the same manner as violations of other 3D Dance Academy policies. If violations appear to constitute a criminal offense, as defined by local, state, or federal statutes, the appropriate authorities may be notified.
