You must be 18 & over and have a student attending TKO to register for an account.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities recommend practicing social distancing.
I further acknowledge that TKO Dance Academy has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that TKO Dance Academy can not guarantee that I or my child will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others.
I acknowledge that I must comply with all set procedures to reduce the spread while attending TKO Dance Academy.
I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19 in the last 21 days.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
*I must notify the studio if I, my child or any one I am in closed connection with has contracted COVID-19 after I registered and join the Academy.
*I understand that the studio will have to closed for a minimum of 14 days OR a specific class will have to be canceled if any student (including my child), staff member or any one who has entered the premise has contracted COVID-19.
*I understand that in the event TKO has to close/quarantine for 14 days OR if my class is canceled due to COVID-19 exposure, I will still have to comply with my monthly tuition. There will be no refund or discount.

I hereby release and agree to hold TKO Dance Academy harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself, my child and/or property that may be caused by any act, or failure to act of the Academy, or that may otherwise arise in any way in connection with any services received from TKO Dance Academy. I understand that this release discharges TKO Dance Academy from any liability or claim that I, my child, or any personal representatives may have against the Academy with respect to any bodily injury, illness, or property damage that may arise from, or in connection to, any services received from TKO Dance Academy. This liability waiver and release extends to the Academy together with all owners.

I, do now release, acquit, and forever discharge the TKO Dance Academy, here referred to as the owner of 99-16 98th Avenue, Ozone Park, NY, 11416 and all of its employees or staff from all actions, claims, demands, or damages occurring to my child resulting from any known or unknown injury, loss, or damage, sustained by my child or any other person or thing in an accident on or related to the above described premises. The acceptance of money in full accord and satisfaction of, and in compromise of a dispute claim, and the payment of money is not an admission of liability on the part of any owner, but is made for the purpose of terminating any dispute and litigation between the parties.

First month tuition is due upon registration. All other monthly tuition will be due 30 days after registration. A Late Fee Penalty will be applied after the 30 days. Failure to pay your monthly tuition after 60 days will result in removal from the class. Tuition is NON-REFUNDABLE
