I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Indianica Academy Inc., its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Indianicaâ€.)
1. I hereby agree to release Indianica Academy Inc. and Indianica harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgment of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
2. I also give Indianica permission to use my child’s picture in or on any form of advertisement for Indianica or an Indianica affiliated event. If I am a minor, my parent and/or legal guardian has also signed this document releasing Indianica from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The participant has my permission to participate in Indianica events and shows.
I warrant the below information is complete and correct. I further release Indianica Academy Inc. of all liabilities associated with my child’s attendance at Indianica.
3. All dance students/parents are requested to strictly follow the daily class dress code as recommended.
4. The due date of tuition fee is determined and scheduled on the first day of attending class by the student. Refund of the tuition fee is allowed only if student/parents cancel the membership in writing within three days of the registration without attending class; after that a minimum of one month tuition fee will be charged. The Registration Fee is non-refundable.
5. The tuition fee is to be paid in advance (up-front) every month on the scheduled due date. Those who are scheduled for special promotional fee are also requested to strictly follow the date of payment. If the fee is not received within three (3) days of the scheduled date, No Promotional Discount will be allowed and the full tuition fee will be charged.
6. Financial assistance and/or Discount in tuition fee granted to two or more students of one family will remain in force as long as there are two or more students of the family are attending. In case only one student continues and the other quits; the remaining one student will pay the full fee without any further discount.
7. The tuition fee must be paid monthly and shall not be affected by lesson schedule and/or attendance. In case students miss a session or two or more in a month they can cover up the lost time by devoting extra time in the next session (Rehearsals for all performances would be considered as cover-up sessions) OR by attending extra class in the coming 4(Four) weeks, but the monthly tuition fee shall not be affected and it has to be paid on the due date each month. Students who attend Rehearsals only are also considered and counted as regular classes. Please further understand that failure to complete the lessons does not relieve the students/parents of their obligation to pay the tuition fee in full.
8. If students fail to cover up lost time in those 4(Four) weeks, it will be their loss and those sessions will be treated as forfeited. The institution is not liable to compensate it in any way whatsoever. Further understand that cover-up classes for missed sessions are allowed only in the months for which the fee is paid. Once the student stops paying regular fee; cover-up classes for missed sessions shall not be allowed after the due date of the fee.
9. In case of students taking a long break and/or plan to go on long vacation of minimum of 1(One) month or more, they must inform the management at least 15 (fifteen) days in advance and they will not be charged fee for the actual time of absence. If they are absent without informing the management in advance as mentioned above, they will have to pay fee for the period of absence along with the fee payable on coming back to the class. Absence of less than one month, DOES NOT include in this category. Para #6 above will apply in such cases. If the long absence is for 3(Three) months or more the student will have to apply for new admission/registration.
10. Indianica remains closed for 2(Two) weeks of holidays in a year – One Week after the annual function of the institution AND One Week of Xmas and New Year from December 24 till January 01 – and these holidays will not alter the monthly payment of fee. In other words the fee has to be paid for these holidays too.
11. All those students who attend the class regularly for more than 8(eight) months in one year starting from July to June the following year (without long absence), will have to participate in the annual function of Indianica.
12. A minimum lump sum amount of $550 per item OR a pro rated amount of $125 for dance lessons and $85 for music lessons per hour, will be charged to take special lessons (for special item/s only). Regular students are exempted from this rule and they get this facility free of charge. The students on long break of one month or more will not get the advantage of the above mentioned free facility during the period of absence; and yet they intend to learn some special item during the break period, they will be charged as mentioned above.
13. If a student is unable to attend for at least one month during the terms due to confirmed health reasons and has paid fee for 3 months, the student has a right to an extension of the arranged payment.
14. If Indianica is closed for 1 month or more, students are entitled to their choice of either extension of the contract or prorated refund, except if the closing is not the fault of the facility, in which case the choice is of Indianica.
15. No pets are allowed in Indianica premises.
16. Please comply with all the rules and regulations of the institution and the traditions of the Indian arts & culture.
