Waiver and Release I agree to participate with ATTD’s program as follows: I recognize the risk of injury common to any performing arts program, and that I am participating in this program upon the express agreement and understanding that I am hereby waiving and releasing the studio and it’s staff from and against any and all claims, actions, and causes of action, damages, costs, liabilities, expenses of judgements, including attorney’s fees and court costs arising out of my participation with this program. I hereby understand and will comply with all policies regarding the studio policies and information. I understand fully that a registration fee along with the first month’s tuition is due upon registering. I understand that my account will be considered delinquent after the 10th of the month and that a $15.00 late fee will be added to my next month’s statement. I understand that a student’s absence will not waive this late fee. I understand that if i withdraw my student from the studio that i must pay the tuition of the current drop month regardless of day. I understand that the costume and recital fees are in addition to normal monthly tuition and are non-refundable. I give ATTD permission to use my child’s photo or video imaging for advertising and marketing purposes. I understand that if my account is not in good standing two weeks prior to a performance that my child will not perform. I also understand that there will be no refunds given. I have read the Waiver and Release, as well as the Dancer Handbook and understand them completely.
