Consent for Service During the Covid-19 Pandemic

Here at Cumberland Dance Academy your well-being and that of our staff is of our utmost importance. In addition to our standard protocols for studio cleanliness, we are taking additional precautions to ensure the safety of our students and staff.
We appreciate and request your help in maintaining a safe environment for yourself and fellow patrons.
As such, we humbly request that you sign this consent form acknowledging you, or anyone that you are in close proximity with, are not currently exhibiting signs or symptoms of Covid-19 and that you understand that though we are taking all precautions to ensure your safety, there are inherent risks to receiving service today.

This is acceptance to allow Cumberland Dance Academy to use pictures and video of your child/children in class or other special events on social media for marketing purposes only.

In the event your child/children are injured on the premises, Cumberland Dance Academy is not responsible. By reading and enrolling your child/children into Cumberland Dance Academy, you agree to these terms. In the event that your child is injured at an event off of the premises of Cumberland Dance Academy, Cumberland Dance Academy is not responsible.
