Waiver of Liability / Acknowledgment of Risk / Photo & Video Release / Acknowledgment of Civility Statement

I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury and, in rare circumstances, death. I agree to release and hold harmless Evolution Performing Arts Center LLC, including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Evolution Performing Arts Center LLC liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes including workshops, rehearsals or performances.
Furthermore, I understand that I should be aware of my, and/or my child’s physical limitations and agree not to exceed them. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.
Further, I authorize and agree that Evolution Performing Arts Center LLC may take and use photographs/videos of me and/or my child for purposes of record keeping, advertising, and marketing. I understand that I do not have any rights to these photographs/videos and will not be compensated for the same. If you wish to opt out, you must submit a written statement to Evolution Performing Arts Center LLC and your request will be taken under consideration.
Evolution Performing Arts Center LLC has also adopted a Civility statement, which reads as follows: “All students have paid to enroll in the class. Inappropriate or uncivil behavior reduces the time all students have purchased to learn and enhance their skills and techniques. Their inappropriate/ uncivil behavior will not be permitted, allowed or tolerated. Members of Evolution Performing Arts Center LLC, which includes faculty, staff, students, and parents are expected to act honestly and responsibly in all areas related to the studio. Evolution Performing Arts Center LLC holds all individuals accountable for their actions and words. Therefore, all individuals should conduct themselves in a manner that acknowledges personal respect and demonstrates concern for the person’s dignity, rights, and freedom of every member of the Performing Arts community, including respect for Studio property and the physical and intellectual property of others. A violation of this policy would be addressed in a meeting with the appropriate individuals in attendance.â€
