Liability Release Form

I understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of the performing arts. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Lisa Ferlita, Stagelights Performing Arts Center, Broadway Magic Foundation Inc., and all affiliated instructors and board of directors involved.

I hereby agree to release Lisa Ferlita, Stagelights Performing Arts, non-profit Broadway Magic Foundation Inc. and hold Lisa Ferlita, all affiliated instructors, and all affiliated board of directors involved harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in the performing arts on behalf of the participant.

I am aware of this release of liability and the acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
If I am a minor, my parent and/or legal guardian has also signed this document releasing Lisa Ferlita, Stagelights Performing Arts Center, non-profit Broadway Magic Foundation Inc, and all affiliated instructors and board of directors involved, from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.

The participant has my permission to participate in classes taught by Lisa Ferlita and the staff of Stagelights Performing Arts Center and Broadway Magic Foundation Inc. I further release Lisa Ferlita, Stagelights Performing Arts Center, Broadway Magic Foundation Inc., and Broadway Magic Foundation Inc. Board of Directors of all liabilities associated with my child’s attendance.

Recital at Stagelights Performing Arts Center is an amazing experience that your child will always remember! Our shows are state of the art and are filled with the joy of dancing, singing and acting!

Recital is optional. We hope your child will participate but also understand that some students want to take class only and opt out of performing at the recital. Please notify us, upon enrollment, by email, if your child will opt out of the recital.

Recital participation includes a costume fee and a recital fee. Costume fees and recital fees are due November and December of 2024. Each class that your child is enrolled in will have a costume fee. The rates for each costume are listed below.
