The 2024-2025 dance season runs the entire school year, beginning in September and ending with the Spring Recital in May.
We require written notice of dropped classes during the School Year Season before the 15th of each month to adjust your billing. If notified after the 15th, membership fees will still be billed for the following month. Paid membership fees are non-refundable.

Liability Waiver

Liability Waiver:

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Dance United and Health Encompassed, LLC.

I/we agree to release and hold harmless Dance United and Health Encompassed, LLC including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Dance United and Health Encompassed, LLC liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Dance United and Health Encompassed, LLC.

I understand that Dance United and Health Encompassed, LLC are insured organizations. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the owner, instructor or staff member as soon as possible.

Photo/Video Waiver

Photo & Video Release: I give permission for Dance United, Health Encompassed LLC, and others assigned by them to photograph and video tape me, or my child, during classes, photo shoots, rehearsals, and performances. These photographs and videos may be used for advertising on the internet, in printed form, or in video form.
