I hereby register my child for ballet lessons offered by Okinawa School of Performing Arts, and certify that my child is physically able to participate in this activity. I fully release Okinawa School of Performing Arts, its employees, owner, Camp Foster, the DOD, USMC, authorized agents, members and/or authorized guests, from any and all claims of injuries, accidents and/or losses I or my child may receive or sustain while learning, practicing or preforming dance movements and techniques during this optional activity.
I understand I must remain present on site for the entirety of my Childs class. I understand the instructor will not be able to assist my child in going to the restroom, and I must be available to escort my child to the restroom.
I understand at no point in time may my child be unescorted anywhere in the gym.
I understand my child, myself and all others accompanying us must adhere to all COVID mitigation procedures, including wearing a mask at all times during class and in the gym.
OSPA Policies
Please read our policies below . Upon enrolling your dancer you agreed to follow all of the OSPA terms and conditions below. These policies are strictly enforced to maintain a safe environment for our dancers and instructors.
1. Compliance with OSPA Policies. Participant agrees to, at all times during the Activity, fully comply with the policies and procedures of OSPA disclosed herein and on OSPAs website, as such policies may be amended, supplemented or otherwise modified from time to time by OSPA in its sole discretion. These policies and procedures include, without limitation, OSPA Participant Drop Policy, Make up Policy, What to Wear, Parent Observation Policy, Class Behavior and Well Policy.
2. DROP PROCEDURE. PARTICIPANTS MUST NOTIFY OSPA IN WRITING TO DROP A PARTICIPANT FROM CLASS AND/OR OSPA ACTIVITIES. The drop notice must be submitted no later than 30 days prior to the following months billing cycle. The Participant is solely responsible for a failure to follow drop procedure.
3. WHAT TO WEAR.
Female dancers in Primary Ballet/Tap: Pink leotard, pink or flesh colored tights, pink or flesh colored ballet shoes, black tap shoes. Hair in a secure bun.
Male dancers in Primary Ballet/Tap: Athletic shorts or pants and a tucked in shirt, black ballet shoes, black tap shoes.
Female Tap/Jazz/Lyrical dancers: Leggings/Biker shorts, tight fitting athletic top OR a biketard OR a leotard and tights, tan jazz shoes, black tap shoes.
Female Tap/Jazz/Lyrical dancers Level 3 and up: Leggings/Bikers shorts, tight fitting athletic top or a biketard OR a leotard and tights, Bloch Socks, tan jazz shoes or foot paws. Black lace up/slip on tap shoes. Hair off the shoulders, neck and out of the dancers face.
Male Tap/Jazz/Lyrical dancers: athletic shorts or pants, tucked in athletic top, jazz shoes, black tap shoes.
Female Dancers Ballet 1-6: Black leotard, pink or flesh colored tights, pink or flesh colored ballet shoes. Hair in a secure bun.
Male Dancers ballet 1-2: Athletic shorts/pants, tucked in athletic shirt, black ballet shoes.
Male Dancers Ballet 3-6: Male black tights, dance belt, white tucked in dance shirt, black ballet shoes.
Male dancers hair standards: please have hair off the face, neck and shoulders.
*People of color may choose to wear flesh toned ballet shoes and tights if they would like to*
4. PARENT OBSERVATION POLICY. Parents must remain onsite in the hallway able to see their dancer at all times. No exceptions unless your dancer is 15 years old or older. Parents MUST accompany their dancer to the restroom, teachers will not be able to take the dancers to the restroom.
6. Class Behavior Standard. OSPA highest priority is the safety and welfare of all Participants in our programs and Activities. OSPA will recommend a different class for a Participant when their behavior compromises their own safety and/or the safety and enjoyment of the other Participants in the Activity/class. The following is non-exhaustive list of behaviors that may subject a Participant to a class change in the sole discretion of an OSPA instructor if the Participant exhibits during an Activity:
Pushing and or Hitting;
Biting;
Spitting;
Foul language and or inappropriate body gestures;
Running away from the group/instructor and not responding to requests to return;
Impulsive behavior that endangers the safety of themselves or others; and
Damaging OSPA equipment, accessories and or property.
