By enrolling my child at AMPA I agree that I am the legal guardian of the child and will be responsible for registration fees, monthly tuition, costume fees, and recital fees. Should AMPA be closed due to a government mandate, I understand that I will still be charged 50% of the tuition fee each month in order for my child to maintain her space in the class upon reopening. I agree that should my child no longer want to attend classes that I will submit a written notice of withdraw to the studio either by email or to the front desk. I agree that should my child withdraw from classes my credit card on file will be charged the remaining balance on my child's account at the time of withdraw.

I understand that my child is being enrolled into a program that uses physical activity. I have agreed that my child is in good physical condition and does not suffer from any condition that would prevent or limit participation in the program.

I understand that I am to assume full responsibility for all risks of physical injury from participation in our program.

I am aware that this is a release of liability and understand all risks to my child in connection with any instruction and further release AMPA and it's employees from liability for any injury or illness sustained while on property or participating in off property competitions and performances with AMPA.

I waive all causes of legal action of any kind against AMPA or any of it's employees. This includes all liabilities, claims, costs, expenses, judgements, attorneys' fees or court costs for any occurrences in connection with AMPA.

I understand that AMPA is not responsible for any child under my supervision who is left unsupervised in any outside areas of the studio including the lobby area. I understand that AMPA will only supervise my child/children when he or she is participating in scheduled dance instruction, activities or programs.

I understand that AMPA is not responsible for personal property that is lost, damaged, or stolen while I or my child is on the AMPA Studio premises.

I acknowledge and agree that it is my responsibility to acquire and maintain my own insurance coverage that provides coverage for myself and my child / children participating in AMPA activities. I understand that AMPA does not provide accident or health insurance for its participants and guardians of participants.

I am aware that all tuition and fees will be charged to the credit card / bank account I have entered in my Parent Portal Account, on the 1st of each month. If the credit card is declined, AMPA will notify me of the decline. If a new card is not entered and payment is not made by the 10th of the month, a $10.00 Late Fee will be assessed to my account.

I am aware that, due to Kentucky state law, that AMPA must collect a 6% sales/service tax on all services beginning January 2023.

I am aware that should my account become 30 days delinquent and I have not contact AMPA regarding setting up a payment plan my credit card on file will be charged the full balance on my account. If the charges are unable to be processed for any reason, AMPA has the right to withhold all dance services until the balance has been paid in full.

I am aware that should the studio close due to government mandate, my account will continue to be charged 50% of the monthly tuition fee, until the end of the season, in order to secure my child's place in class upon re-opening.

Should my child withdraw from classes, I agree to contact the studio via email no later than the 15th of the month in order to suspend any future auto-payments. I also agree that the remaining balance on my account will be charged in full upon receipt of the email request to withdraw.

I am of lawful age and competent to agree to this waiver.

By proceeding with registration and payment I am agreeing to the above policy for me and my child / children.

I have received or will obtain a copy of Academy of Movement and Performing Arts (AMPA) policies and will take the responsibility to carefully read and follow the rules and policies therein.
I understand that Academy of Movement and Performing Arts (AMPA) does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. Should the studio be forced to closed due to government mandate, I understand that auto-payment will still be processed at 50% of my monthly tuition cost to hold my child's space in class.
I understand that costume payments are final and no refunds are given after payment is taken. Should your child withdraw after costumes are ordered they may have their costume but no refund will be rendered.
I understand that that recital fees are due from all students/families on the given due date. This is not an optional fee.
I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Academy of Movement and Performing Arts (AMPA), and I voluntarily assume the risks associated with such participation and agree to hold AMPA harmless should any injury, or damage of property occur.
