Must be over 18 years of age to register an account.

The agreement is the participant is in good physical health and capable of performing the physical movement and of the exertion involved in the classes and serves to waive all liability against the Illustrated Movement Dance Academy and its staff and representatives for any injury or loss of property.

Illustrated Movement reserves the right to use photos and videos of students from dance classes and performances for advertising and marketing purposes. To opt out of the photo/video media release, please accept this waiver and continue to our "photo opt out" or contact the front desk to opt out of photos

Fees are due on the 25th of the previous month, every month (example, Sept. tuition is due August 25th). Late payments of fees (posted on the 2nd of each month) will have an added late fee of $35.00 on the total amount owed. After the second reminder classes will be suspended until payment is made. Credit cards with insufficient funds and/or declined will be charged $20.00 + all late fees. Classes are ten months/34 weeks in length, tuition is broken down into monthly payments. For summer programming, tuition is due upfront.

Withdrawal from classes: The office must be informed 30 days in advnaced and a student withdrawal sheet must be completed by the parent/guardian. Informing the teacher, phone calls and emails are not accepted forms of withdrawal from classes. The only acceptable method is the completion of a student withdrawal form at the office. The signed date on the student withdrawal form will be the date used for any refund. The administration fee of withdrawal is $35.00; if the withdrawal happens after choreography has been set (December - July), there is a $100.00 re-choreographic fee. Registration fees, costume deposit, costume fees, booster club, competition fees (if on the competitive team) exam fees (if taking the exam) are non-refundable.

Payment is due by credit card or e-transfer on the 25th of the prior month each month. Late payments: Accounts will be charged $35.00 after the 10th. Cards declined or insufficient funds for any reason will be charged an additional $15.00. Classes will be suspended only when the account is up to date unless a payment arrangement has been agreed upon.

Withdrawal from any program requires 30 days' notice (one month's tuition). A completed withdrawal form is required. Withdrawal by email, text, phone, or person will not be accepted.

By checking I, the undersigned, give permission for Illustrated Movement Dance Academy to use video footage and / or photographs of my child/ward. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:

- printed materials (e.g. - brochures and newsletters)
- online and offline advertising and promotion
- Videos and digital images are for use on social media.

By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Illustrated Movement Dance Academy for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
