Liability Waiver
Medical Consent, Photo Permission, and Liability Waiver
1. I understand there is an inherent risk associated with any exercise program that may result in injury. I understand and am aware that the components of exercise are potentially hazardous activities and may cause injury.
2. I acknowledge that I and/or my child(ren) have either had a physical examination and/or have been given permission from my physician to participate in an exercise program or that I and/or my child(ren) have decided to participate in an exercise program voluntarily and without the approval of my physician and do hereby assume all responsibility for my or my child(ren)’s participation in any exercise program, instruction, choreography, performance and/or event associated with MoveStudio Charlotte, LLC.
3. I certify that I and/or my child(ren) are physically well and suffering from no medical problems, conditions, impairments, diseases, or any other illness that would prevent my or my child(ren)’s participation or increase my or my child(ren)’s risk of injury and/or illness as a result of partaking in any exercise program, instruction, choreography, performance and/or event at MoveStudio Charlotte, LLC.
4. I, my heirs, or legal representatives, do hereby waive and release MoveStudio Charlotte, LLC, its teachers and employees from any and all liability and responsibility from injury, accident, illness, legal and medical fees sustained now or in the future resulting from my or my child(ren)’s participation in any activity or use of equipment.
5. I agree that MoveStudio Charlotte, LLC is in no way responsible for the loss or damage of my or my child(ren)’s belongings while attending any exercise program, instruction, choreography, performance and/or event.
6. I agree, unless I explicitly request otherwise, that photographs taken of me or my children during this program may be used for promotional purposes by MoveStudio Charlotte.

In checking the box below, I acknowledge that I have read this waiver of liability
I fully understand its terms and conditions, and understand that I am giving up my right to sue MoveStudio Charlotte, LLC, its teachers and staff, and completely and unconditional release MoveStudio Charlotte, LLC of liability to the greatest extent allowable by law.
