Financial Responsibility

I accept full responsibility for all related cost during 2024/2025 season at Stages Dance Academy.
These costs include registration, monthly tuition, late fees (if applicable) and costume expenses. Costume
charges and deposits are an addition to tuition charges and both are non-returnable and non-refundable. I understand
that if my account becomes delinquent, the dancer will not be able to participate until the
account is current, and I am fully responsible to pay the balance due. I understand all payments are
non-refundable and the monthly payment is for the entire month. If you do not attend class, NO REFUNDS
or discounts will be given. I agree that all classes will be paid in full participate in the dress
rehearsal and dance recital for the Winter (if applicable) and Spring Show.

REGISTRATION FEE: $30 PER DANCER or $35 FAMILY

45 min________$55 Mo. $495/Yr *2nd class/child $47mo. $423/yr *3rd + $40 Mo. $360/yr
1 Hr___________$60 Mo. $540/Yr *2nd class/child $51 mo. $459/yr *3rd + $44 Mo. $396/yr
Tap I or Pointe IV Add On- $30 mo. $270/yr (add on applied to 1st class)

6 Week Session Class (Tot Dance) ______ $75 per 6 week session
Private Lesson: 30 Min______$30 a class 1 Hr________$60 a class

*Multiple class discount applies to same dancer and/or siblings.

Choose from the following:
______Full Tuition for the year due on registration day. (One Payment with a 5% discount)
_____Monthly (9) payments. First payment on registration day and monthly payments on the first class of every month-May.
*A $20 fee for late payments not received by the 15th of the month and a $25 fee for each returned check*

Student Release

I assume full responsibility and risk of bodily injury for the dancer enrolled at Stages Dance Academy. Our goal is to provide a safe and accident free environment: however in the event of any bodily injury, I release Stages Dance Academy and the instructors from all liabilities. I understand I am not able to participate if I have COVID and/or am sick with a fever. I have read and voluntarily sign this release form.
