Shoreline Dance Academy, LLC takes every precaution to ensure your child's safety as we are committed to anatomically informed and injury free dance. Yet, as dance is a physical activity, injuries may occur. SDA instructors are not responsible for injuries occurring before, during, or after class. Each student may decline to participate in any activity which they deem to be harmful and must inform the instructor of any physical limitations that may prevent full participation in class.
I hereby declare that I have provided and will continue to provide adequate and complete health insurance coverage to protect my child in case of injury or accident. Such insurance includes coverage for any and all activities in which my child will participate with SDA. I understand and agree that SDA does not, and cannot, assume any financial responsibility for any medical assistance or care my child may require.
I am aware that dance programs are to be considered strenuous physical activities. Therefore, I have received the consent of my physician for myself or for my child to participate in this program.
I have read this fully and understand the above written statements.
Policies
MONTHLY CONTINUOUS ENROLLMENT: Payments are made monthly and you do not have to worry about your spot being taken. You are guaranteed your spot all year as long as you do not withdraw and fees are up to date.
Our monthly tuition payments are based on an average of four classes per month. No additional charge is assessed for months that offer five classes. In months when holidays, studio closings, snow days, etc. affect the schedule of classes, no tuition is reduced. Tuition is not prorated for absences. Payments are due at the time of registration.
PAYMENT POLICIES:
• Each family is required to have a credit/debit card on file for payments.
• Tuition is paid monthly: Tuition is due on the 25th of each month for the following month.
• Options for making payments before the 25th of each month:
If payment is made prior to the 25th of the month by one of the options listed below then no fees will be charged to your credit/ debit card on file.
1. Payments may be made in person with cash.
2. Payments may be made with a personal check. Please make your check payable to SDA. A $25 fee will be assessed on all returned checks.
3. Payments may be made with VISA, MasterCard, American Express, or Discover.
4. You may pay online through paypal.

6. Remember, any remaining balances at the end of the day on the 25th of the month will be charged to your credit/ debit card on file. This is done automatically so you will not have to worry about losing your child's spot in class.
If there are changes to your credit/debit card the new information must be received by the 20th of the month. If your payment doesn't go through for any reason you will be charged a $15.00 decline/late fee. If delinquent tuition is not paid before the first of the month, your child(ren) will be tagged inactive by the computer, will be un-enrolled, and children on the waiting list will be called. Once we have received a bad check, all future tuition must be paid with cash or credit/debit card.
WITHDRAWAL POLICY:
If you need to become inactive in the program, a Withdrawal Form, located in office, MUST BE received by the 20th of the month prior to withdrawal. WE DO NOT ACCEPT WITHDRAWALS OVER THE PHONE OR THROUGH EMAIL. WE MUST RECEIVE A PHYSICAL COPY OF THE WITHDRAWAL FORM in the office.
Please Note:
UNLESS WE RECEIVE PROPER NOTICE, YOU WILL BE RESPONSIBLE FOR THE NEXT MONTH'S BILLING TUITION.
ANNUAL REGISTRATION FEE:
This fee is $25 for the first child or a $50 maximum per family. Registration is due at the time of initial enrollment and renewed annually each year thereafter. This fee covers the cost of insurance as well as registering and maintaining family records.
FAMILY DISCOUNT:
We offer a generous discount for multiple classes for one child and multiple children from one family. There is a 10% Discount for each additional member of the immediate family calculated on the lower priced class(es) for all programs at Shoreline Dance Academy. This does not include private lessons.
PAYMENT TYPE:
We accept Cash, Check, Visa, Discover, American Express, and Master Card.
MINIMUM CLASS SIZE:
All classes must have at least 4 students.
REFUND POLICY:
We do not offer credit and/or refunds for cancelled classes due to holiday, vacation, illness, weather or any other reason. Shoreline Dance Academy does not issue refunds. All sales are final.
Performance fees not included.
SDA will be closed if South Haven Public Schools are closed due to weather conditions.
Students must adhere to SDA dress codes and hair must be pulled back from face and off the neck.
No food, candy, or drinks (other than water) are allowed in the facility.
