You represent (1) that you understand that participating in Take A Bow LLC programs, which include voice, acting and dance instruction, include an inherent risk of injury: and (2) that your child is in good physical condition and has no disability, illness or other condition that could prevent him/her from participating in activities without injuring him/herself or impairing his/her health. If your child has any special exercise requirements or limitations, you agree to disclose them to Take A Bow LLC before enrolling your child. You, individually and on behalf of your child, hereby release and hold Take A Bow LLC harmless from, and hereby assume all responsibility for, all claims, demands, injuries, damages, actions, or causes of action whatsoever, as well as any exposure to an infectious disease, including Covid-19, arising out of, or connected with, your child's participation in Take A Bow programs, except as such may arise from the gross negligence or willful misconduct of Take A Bow LLC, its employees or agents. Additionally, you give permission for Take A Bow LLC to take photos of your child to use for purposes of promoting the school and waive any rights to compensation therefore. Class schedule is subject to change based on registration numbers. If the revised schedule is not acceptable, you will receive a full refund of all monies paid. REFUND POLICY: Deposit is refundable minus a $30 admin fee until the first day of the semester. Once the semester begins, the $100 deposit is non-refundable. Full tuition must be paid by the start of classes. There is a 10% late fee applied for payments received after the first week of classes and a $25 fee for all returned checks. No refunds will be given once a child enters their third class or once we reach the fifth week of the semester. 24-hour notice is required for cancellation of a private lesson in order to avoid a fee. Tuition is non-transferable.
