North Shore Dance and Tumble Academy has put in place preventive measures to reduce the spread of COVID-19; however, the studio cannot guarantee that you will not become infected with COVID-19. Further, attending the Studio, participating in classes, rehearsals and other studio activities could increase your risk of contracting COVID-19. By signing this agreement, I/my child acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I/my child may be exposed to or infected by COVID-19 by attending the classes/activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I understand that I consent to the use of photos and videos taken at North Shore Dance and Tumble Academy or any event sponsored by North Shore Dance and Tumble Academy to be posted on social media or to be used on the company website or for advertising purposes unless I 'opt-out'. No names will be included in any public use of these photos and videos.

This agreement releases North Shore Dance and Tumble Academy, LLC from all liability relating to injuries that may occur during any activity, on the location of, and while participating or not, in dance and tumbling classes. By pressing "I Agree" to this agreement, I agree to hold North Shore Dance and Tumble Academy, LLC entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

I also acknowledge the risks involved in participating in dance and tumbling classes. These include but are not limited to personal injury through activity and use of equipment. I swear that I am participating voluntarily and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

By pressing "I Agree" below, I forfeit all right to bring a suit against North Shore Dance and Tumble Academy, LLC for any reason. In return, I will receive dance and tumbling instruction. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

By pressing "I Agree" below, I am confirming that I have carefully read and will follow the rules and policies therein on North Shore Dance Academy's website. I understand that North Shore Dance and Tumble Academy does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I understand that I may incur late or drop fees corresponding to the policies listed on the company's website. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with North Shore Dance and Tumble Academy, and I voluntarily assume the risks associated with such participation.
