Commitment to Beating Coronavirus and Providing a Safe Studio for our Dancers

Empire Premier Academy is dedicated to doing our part in keeping our dancers and the studio as safe as possible during this unprecedented time. Please see our policies and procedures that we will be following and need your support in following to ensure we can continue to do what we love the safest way.

• All dancers will be required to have their temperature taken when they enter the building. If your dancer has a temperature of 99.7 degrees or higher, they will be asked to leave and they will need to be temperature free for 3 days before returning to the studio. If you dancer develops any symptoms associated with coronavirus, we may ask that they have a test to rule out coronavirus before they are able to come back to the studio.
• All dancers and staff will need to wash their hands when they enter the studio and several times during practice.
• Anyone who is feeling unwell or displaying any symptoms associated with covid-19 is not allowed to enter the studio.
• Masks will be worn by all staff and coaches while teams are practicing at the studio
• All common surfaces and common areas will be sanitized and cleaned after each use or several times each day. We will ask our dancers to sanitize and clean all equipment they use after they are finished with it. Please talk to your dancers about this and help them understand how important this is.
• For the time being, we are asking that parents/guardians to not wait in our lobby and only come into the studio to drop off and pick up your dancer. If you do need to come in during practice, we are asking that you maintain physical distancing guidelines. The staff will make sure all dancers get to and from practice.
• If anyone in your household or anyone you have come in contact with has been confirmed to have coronavirus, you will notify the studio immediately.
• You are committing to Empire Premier Academy knowing the inherent risk of exposure to COVID-19 and that it could exists in any public place where people are present. By committing to Empire Premier Academy, you voluntarily assume all risk related to exposure to COVID-19.
• Empire Premier Academy will reimburse dancers competitions fees for any competition that is cancelled due to coronavirus and cannot be rescheduled or replaced. If competitions aren’t cancelled, you will be required to attend the competition. We will do everything we can to minimize the time spent at a competition.

WAIVER Empire Premier Academy, Empire Premier Academy Participant Agreement, Release and Assumption of Risk

In consideration of the services of Empire Premier Academy, it's owners, agents, officers, subcontractors, employees and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as "EPA"), I hereby agree to release, discharge and hold harmless EPA, on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows: 1. I understand and acknowledge that the activities that I or my child engage in while on the premises or under the auspices of EPA pose known and unknown risks which could result in injury, paralysis, death, emotional distress or damage to me, my child, to property, or to third parties. The following describes some, but not all of those risks: Dance and gymnastics, including performances of stunts and use of trampolines, entail certain risks that simply cannot be eliminated without jeopardizing the essential qualities of the activity. Without a certain degree of risk, dance students would not improve their skills and the enjoyment of the sport would be diminished. Dance and gymnastics expose participants to the usual risk of cuts and bruises, and other more serious risks as well. Participants often fall, sprain or break wrists and ankles, and can suffer more serious injuries. Traveling to and from shows, meets and exhibitions raises the possibilities of any manner of transportation accidents. In any event, if you or your child is injured, medical assistance may be required which you must pay for yourself. 2. I expressly agree and promise to accept and assume all of the risks, known and unknown, connected with EPA- related activities, including but not limited to performance of stunts and use of trampolines. My participation and that of my child is purely voluntary. No one has force or coerced me or my child to participate. I elect for myself and my children to participate in such activities in spite of the risks. 3. I hereby voluntarily release, forever discharge and agree to hold harmless and indemnify EPA from any and all liability, claims, demands, actions or rights of actions, which are related to, arise out of or are in any way connected with my child's participation in EPA-related activities. 4. Should EPA be required to incur attorney's fees and cost to enforce this agreement, I agree to indemnify and reimburse them for such fees and costs. 5. I certify that my child has health, accident and liability insurance to cover bodily injury or property damage that may be caused or suffered while participating in this event or activity, or else I agree to bear the costs of such injury or damage to my child. I further certify that I am willing to assume and bear the costs of all risks that may arise or be created, directly or indirectly, through or by any such condition. In the event that I file a lawsuit against EPA, I agree to do solely in the State of Ohio and I further agree that the substantive and procedural laws in that state shall apply in any such action without regard to the conflict of laws rules thereof. I agree that if any portion of this agreement is found void or unenforceable, the remaining portions shall remain in full force and effect. 7. By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation or the participation of any of my children in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against EPA on the basis of any claim from which I have released EPA by signing this Agreement. I have had sufficient opportunity to read this entire document. I have read it and understand it. I agree to be bound by its terms.


In consideration of____________________ (print minors name ("Minor") being permitted by EPA to participate in it's activities and to use its equipment and facilities, I further agree to indemnify and hold EPA from any and all claims which are brought by, or on behalf of Minor and which are in any way connected with such use or participation by Minor.

I hereby consent to the photography of my child and the recording of my child's voice and the use of these photographs and/or recordings singularly or in conjunction with other photographs and/or recordings for advertising, publicity, commercial and other business purposes. I understand that the term "photograph" as user herein encompasses both still photographs and motion picture footage. Further, I understand others, with or without the consent of Empire Premier Academy, LLC may use and/or reproduce such photographs and recordings. I hereby release Empire Premier Academy, LLC and any of its associated or affiliated companies, their directors, officers, agents, employees and customers, and appointed advertising agencies, their directors, officers' agents and employees from all claims of every kind on account of such use.
