ASSUMPTION OF RISK, WAIVER OF LIABILITY AND INDEMNIFICATION AGREEMENT: I am aware that dance, acting, and musical theater, and the exercises associated with it, can place stress on the body, and carry with it the risk of physical injury. On behalf of my child and myself (and if I am no longer a minor, on my own behalf), I assume the risk and agree that Cumberland County Playhouse and T3 Education Program shall not be liable in any way for injuries sustained during attendance at Cumberland County Playhouse and T3 or any of its related functions. Student or Guardian/Parent of Student understand and acknowledge that Cumberland County Playhouse and T3 is not insurers of Student’s behavior, actions or participation in the program/classes, and that Cumberland County Playhouse and T3 assumes no liability whatsoever for personal injuries or property damages to Student or to third persons arising out of participation in the program activities, classes, or any activities related thereto. Student or Guardian/Parent hereby agree to release, waive, covenant not to sue, indemnify and hold harmless Cumberland County Playhouse and T3, its owners/directors, teachers, and employees from any and all liability, claims, demands, judgments, actions, executions and causes of action whatsoever. I UNDERSTAND THAT I AM WAIVING MY RIGHT TO TAKE LEGAL ACTION INCLUDING FILING A LAWSUIT FOR PERSONAL INJURIES TO MY CHILD(REN) AND/OR MYSELF.

I hereby enroll the above student for and agree to pay all (non-refundable) tuition and fees on a monthly basis in accordance with the policies. I agree to enable my child to conform to the dress code as stated. By signing below, I acknowledge that I have read, understand, and accept the payment policies, dress code, and other rules, regulations, and procedure of the Cumberland County Playhouse Education Program. I also understand that for the safety of the students, my child must be picked up inside the building and not be allowed to wait outside for a ride after his or her classes.

I hereby grant full permission to the Cumberland County Playhouse and T3 to use my and/or my child's likeness or photo or any other record of this event and related activities for legitimate purposes.
