By checking this box you acknowledge that you have read and understood ALL fall information available on our website, www.ckda.com concerning the cost and requirements for enrollment at CKDA.

You acknowledge that you understand if a costume is ordered for your child you are responsible for the costume fees required for your child's class.

You understand that costume fees are due by December 1st.

You understand that you are responsible for a $50 recital fee for the first child and a $10 recital fee for each additional child, due in April.

You understand that you must drop a class by emailing LexiCKDA@gmail.com BEFORE the 1st of the month or you are responsible for that month's tuition.

You understand that your child's name/picture/video may be used for marketing purposes when in class at CKDA, at CKDA functions, participating in an event with CKDA, etc.

You are giving permission for your child to attend classes knowing that it is possible that they could come in contact with COVID-19 and any other ailments will not hold Cathy Kurth Dance Academy responsible.

CATHY KURTH DANCE ACADEMY RELEASE AND WAIVER OF LIABILITY AGREEMENT

I acknowledge that I have voluntarily enrolled my child/children in dance/acrobatic/cheer classes at Cathy Kurth Dance Academy. I am aware that these activities are athletic activities and can be hazardous or dangerous. I am aware that my child could be seriously injured by participating in these activities due to the athletic nature of dance, cheerleading and acrobatics. I am voluntarily enrolling my child/children in these activities, and acknowledge the danger involved and agree to assume any and all risks of bodily injury, death or property damage, whether those risks are known or unknown.

I give permission for my child to attend classes knowing that it is possible that they could come in contact with COVID-19 and other ailments and will not hold Cathy Kurth Dance Academy responsible.

I forever release Cathy Kurth Dance Academy, staff and all instructors from any and all actions, claims or demands that I, my children, next of kin, spouse, guardians, heirs or other representatives now have or may have in the future for injury, death, or property damage related to the activities that I voluntarily enrolled my child/children in.

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability, and a contract between myself and Cathy Kurth Dance Academy, and sign it of my own free will.
