I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Brevard Ballet Academy, it’s owner, faculty, instructors, and all affiliated entities (hereinafter collectively referred to as “Brevard Ballet Academy”).
I hereby agree to release Brevard Ballet Academy and hold Brevard Ballet Academy harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
If I am a minor, my parent and/or legal guardian has signed this document releasing Brevard Ballet Academy from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.
The participant has my permission to participate in Brevard Ballet Academy events. I further release Brevard Ballet Academy of all liabilities associated with my child’s attendance at Brevard Ballet Academy.
•As with any program, steady attendance is a requirement for success. Failure to meet such requirements can lead to adjustment in class level.
•Parents/Guardians are responsible for contacting the dance studio office directly regarding absences.
•Any student that misses two consecutive weeks, without notice, may be required to forfeit their class spot as space is limited.
•Students are allowed one make-up class per month and must be made up within 4 weeks of the missed class.
•Make-up classes must be scheduled and approved by class Instructor.
•There are no refunds nor can missed classes be applied to subsequent months’ tuition.
Schedule Changes or Dropping Classes:
•All schedule changes must be approved by Instructor and Academy Director.
•The office must be notified prior to dropping classes or making schedule changes online.
•All fees are due until the office is notified of any class being dropped.
We welcome you as parents to bring any concerns to our attention. We do ask that you use discretion as you address them with us and that any conversations that are sensitive and private in matter be held while classes are not in session. If issues concern other parents or children, please do not address these issues at the studio. If issues concern faculty, please do not address the issue in front of other students and/or parents. Please resolve the issue independently or make an appointment with the director to resolve the situation. Angry dialog, foul language and/or threats will not be tolerated. Parents and/or students who display this type of behavior will be dismissed. Please remember that these rules are for the safety and comfort of all students and faculty.
Studio Rules for Parents:
•All young children must be supervised. No running, yelling or rough playing at the dance studio.
•To maintain the comfort, privacy, and productivity of our students, our studio policy is that no videos or photos can be taken during class time.
•Help keep your studio clean and tidy. Please throw away all trash.
•No gossip, obscene language or inappropriate conversations are tolerated by either parents or students.
•Promptness is extremely important.
•Dancers need adequate time to be physically and mentally prepared for the demands of the dance class.
•It is recommended that students arrive early to have the time to use the restroom, get warmed up and be dressed properly.
•If you arrive late and your class is in the middle of a combination, please wait until the combination is over to enter the studio.
•Proper dress code is required for all classes.
•Uniform (including shoes) must be clean without any stains, tears or rips.
•Hair must be in a bun and out of face.
•Nails should be cut and clean. No bright colored nail polish allowed.
•No t-shirts, sweat pants or loose clothing worn in class unless specified. Over-sized clothing inhibits the instructor to correct body placement.
•Jewelry should not be worn to class as it may become a distraction or a hazard.
•All ballet shoes should be canvas material. No leather.
•Other (local) studio or company apparel should not be worn to the studio.
We expect all dancers to show respect to themselves, each other and all instructors.
•Be on time and prepared for class.
•Use bathroom prior to class to minimize interruptions during.
•Do not have frequent class absences.
•No gum is allowed in class.
•Do not talk during class unless you are asking a question.
•Be willing to try new dance material. Put positive energy into your dance class.
•No leaning, lounging or hanging on the ballet barres.
•If injured, tell instructor prior to class.
•If ill, do not come to class.
•If you need to leave early, notify instructor prior to class.
•At the end of class all students should thank their instructor with a curtsy.
•When leaving the studio, students must remove dance shoes and cover-up their dance clothes or change. Do not go outside during cooler weather without a warm-up. It is important to control the cooling process to avoid injury. In addition, please respect your art by keeping your dance attire confined to the studio.

•No gum, food or drink of any kind allowed inside the studio rooms.
Tuition and Payments:
• The annual registration fee of $40 per dancer and $15 for each additional sibling is due at the time of registration.
•Our tuition prices are based on a 20-week semester, already accounting for holidays and breaks.
•Payment is due on or before the 1st of each month.
•Accounts not paid by the 1st of the month are subject to a $20 late fee that will be charged to your portal. Arrangements can be made for special circumstances.
•If your account has any outstanding balances, the student will not be allowed to participate in school productions or events until paid.
•Please check your email and parent portal for current and upcoming balances and statements.
•Tuition refunds are prorated. If you’ve paid for the season, any discounts received will be nullified with the refund.
•No refunds on classes that have already been taken.
•If a student’s account is delinquent for 30 days, they will not be allowed to take class until account is current.
•If a student withdraws from classes and re-enrolls within the year, they must re-register and pay the registration fee.
All parents and students must have an online portal set up. To avoid any errors, all payments must be made using the following options and never handed to a faculty member. *If you choose to opt out of online payments, we must have a credit card on file for any account fees.
•AUTO-PAY: An AutoPay system can be set up on your online portal to debit your checking, savings account or credit card each month. The AutoPay system will debit your account on the 1st of every month. This helpful system will avoid any late payments resulting in late fees.
•CREDIT CARD: Credit card payments must be made online via the parent/student portal. All credit card payments will be charged a 3% convenient fee.
•CHECK: Checks can be mailed or dropped in a filled-out envelope in the payment box located in the front and back lobbies of Studio B. If the name on the check does not match the dancer’s name, please make note on the memo line. A $15 fee will be charged to your portal for all returned checks (NSF).
•CASH: All cash payments may be dropped in a filled-out envelope in the payment box. Please use exact change. Please do not mail cash payments.
