BY REGISTERING FOR CLASSES AT IAM YOU UNDERSTAND AND AGREE TO THE FOLLOWING TUITION, PAYMENT, AND WITHDRAWAL POLICIES:
- All students 6 years & older commit to the entire session ( Summer July 8th - August 16th, Fall September - December or Spring January - June 2024) when they register for class the legal parent or guardian is financially responsible for the tuition of each month of that session regardless of if the student participates in class or not. If the child drops the class mid session, the card on file will be charged monthly for the tuition amount until the end of the session. By registering for classes, you agree and understand that IAM will charge the card on file monthly through the end of the session regardless of if the student participates in classes or not. You agree and understand that an active card must be on file at all times, in the case that the card is declined, cash or check of the total amount due is due to IAM by the due date of all outstanding fees. All tuition, registration, drop-in, participation, merchandise, and costume fees are non-refundable and non-transferable. This allows IAM to reserve the appropriate amount of spaces in classes, order equipment in appropriate amounts, lead students through a structured program throughout the session, and pay our staff a fair wage for their dedication to IAM and it's students.
- By registering for classes you agree and understand that you must add a card on file to your account. You may pay via cash or check, but the card on file will be charged if tuition is more than 7 days late or if the child (5 years or younger) withdraws from the class. During January - June session, costume & spring show fees are still due by their respective due dates and are non-refundable and non-transferable, regardless of the students participation in classes.
I UNDERSTAND AND AGREE TO THE FOLLOWING:
-ALL TUITION AND FEES MUST BE PAID BY THEIR RESPECTIVE DUE DATES. Any accounts that have past due statements will be charged a $10 late fee the day after the due date, and again for every week in which the full payment is not received. (Example: payment is due Monday, May 2nd. If the payment is not paid in full, a late fee of $10 will be charged on May 3rd, followed by another $10 fee on May 10th).
- Accounts in which competition or convention fees are not paid within 5 days of the due date will result in the student being removed from the competition and will not be permitted to compete until all balances are paid in full and until the next competition or convention.
- Any accounts that are more than 5 days past due will be automatically charged the full amount of the balance to the bank account or card on file. I agree and consent that Industry Athletics & Movement, LLC may charge my card or bank account on file for the full amount of the balance owed if it has not been paid within 15 days after the posted due date.
- Tuition for Fall, Winter, & Spring is based on four classes per month. Please note that tuition is not prorated when there are fewer than four classes, and in return, there are no added fees when a fifth class occurs in a month. Tuition is the same each month regardless of the amount of classes that fall during that month.
-Tuition is subject to change
-Tuition is non-refundable. We do not prorate or refund for absences, but we do offer make-ups.
- If the studio unexpectedly closes due to inclement weather, natural disaster, local or national health directives, or any other reason, refunds and credits are not issued for cancelled classes. Students may attend any age appropriate class they choose in the future as a makeup class for the amount of hours of class missed.
- Notice of withdrawal must be written via email to leah@contactiam.com by the 25th of the month for students 5 Years and under in order for the following month to be waived of tuition and fee charges for the following month. The full amount for the current month is non-refundable. Withdrawals only waive tuition for the months following the 25th of the current month. Any withdrawal notice given after the 25th of the month will be charged the full amount of tuition for the following month.
- Students 6 years and older are committed to the entire session. Parents/Guardians of enrolled students are financially responsible for the entire session regardless of participation by the child. See "Academy Structure" waiver for more details.
- I agree and understand that registration fees, costume fees, and any other fee posted to my account must be paid by the due date, and is subject to the policy above. If the student wishes to re-enroll after withdrawal, the registration fees apply upon re-enrollment with a $10 discount. Fees are non-refundable and non-transferable.
- I agree and understand that by participating in the Winter/Spring session, all costume and spring show fees are due by their respective due dates and are non-refundable and non-transferable. Students who have an outstanding balance may not participate in classes or performances.
ATTENDANCE & ETIQUITTE
I AGREE AND UNDERSTAND THAT STUDENTS WHO MISS MORE THAN 3 CLASSES PER MONTH BETWEEN THE MONTHS OF MARCH AND JUNE MUST SCHEDULE A PRIVATE LESSON WITH THE INSTRUCTOR TO KEEP CURRENT ON THE RECITAL CHOREOGRAPHY AT A RATE OF $65/HOUR. INSTRUCTOR WILL SET THE AMOUNT OF TIME NECESSARY.
By signing, you also agree to the below policies:
- Students who miss more than 3 classes between March and June may be subject to removal from the spring show, and are not exempt from withdrawal fees but may continue to participate in class.
- All persons onsite at Industry Athletics & Movement, LLC must adhere to civil, friendly, and respectful behavior at all times. Any rude, aggressive, or hostile behavior by students, their family members, or anyone associated with the student may result in immediate dismissal from the premises and/or dismissal from the program. No refunds or credits of any kind will be administered to anyone who displays any type of the behavior listed above
-Industry Athletics & Movement, LLC is a drug, alcohol, and weapons free environment. Any persons who appear to be in possession of, or under the influence of any of the previously listed items will be asked to leave the premises immediately.
- All students must adhere to the dress code, and may be asked to sit out of class otherwise.
-All responsible parents & guardians of students enrolled at IAM are responsible for reading email, text, and social media communication about events or details that pertain to their student at IAM. IAM is not responsible for parents or guardians who miss or disregard information and/or details that have been communicated by IAM. All parents & guardians must agree and understand that reading all communication from IAM and following directions is a requirement of participating in IAM programs, events, and classes.
-You agree and understand that IAM is drop off only for dancers 6 years and older. This helps us keep a safe capacity in our lobby and keep the flow of traffic manageable.
I AGREE AND UNDERSTAND THAT THE REQUIRED ATTIRE AND DRESS CODE OUTLINED BY INDUSTRY ATHLETICS & MOVEMENT, LLC MUST BE ADHERED TO IN ORDER FOR ANY STUDENT TO PARTICIPATE IN ANY ACTIVITY WITH INDUSTRY ATHLETICS & MOVEMENT, LLC.
- Students may be asked to leave class or sit out if the attire policy is not followed. An email will be sent to the parent or guardian if the attire issue continues after 2 offenses. This is both a safety and industry standard policy.
By registering for classes at Industry Athletics & Movement, LLC you agree and understand that all active accounts must have a current card on file that must be enrolled in auto-pay starting January 1, 2024. This is to protect our business from overdue accounts and helps us ensure we can pay our staff on time. If you pay your bill via cash or check by the 5th of each month, then you will not be charged when we run auto-pay, as long as your account is at $0. Only if your account is past due by the 5th of the month will we run auto-pay, charging your card on file on the 6th of each month for the entire amount due. We appreciate your understanding that we are a small business, and receiving payments on time is the only way we can keep our doors open and staff paid. By registering for classes, and having your dancer participate in them, you agree to place a current card on your DSP account, and understand that it will be charged for your entire amount due if your account is past due on the 5th of any month, and that dancers who's accounts are more than 6 days delinquent may not participate in class until the account is brought to $0.
- I UNDERSTAND & ACCEPT THAT INDUSTRY ATHLETICS & MOVEMENT, LLC IS NOT A REGISTERED CHILDCARE FACILITY.
-I understand and accept that students must be picked up from the facility no more than 5 minutes after the end time of their final class of the day, and be dropped off no earlier than 15 minutes before the start time of their classes. Students who are picked up more than 5 minutes after the end of their scheduled class time will be charged a $10 fee on their account.
-I understand & accept that students must be sent to the facility with water and nutritious food if they are taking 2 or more hours of instruction at Industry Athletics & Movement, LLC. Students who are taking 2 or more hours of instruction and do not have water with them will receive a $2.00 charge on their account and be provided with a disposable water bottle if they are in stock. All students are responsible for providing their own water. Industry Athletics & Movement, LLC is not responsible for any injury or illness that may occur from the provided bottle of water. Under no circumstances may a student share their food or water with another student due to allergy and illness concerns.
- Siblings may not stay on site during the registered students class time or breaks. Industry Athletics & Movement, LLC reserves the right to dismiss any disruptive persons on site.
- Students who are remaining on the premises during break times are not supervised, their parent or guardian is liable for any and all of their behavior, actions, and anything that may occur during their time on the premises. IAM reserves the right to require students to be picked up on their breaks if their behavior is not satisfactory.
PHOTO & MEDIA REALEASE
By signing, you acknowledge and consent Industry Athletics & Movement, LLC to take photos and/or videos of my child/myself and use them in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio’s website, or on social media sites (such as the studio’s facebook or instagram/tiktok account). I understand that in some cases, my child and/or my name may be used. I further understand no financial compensation will be provided for the use of these photos/videos. I release the rights to use any media of myself//my child on the above platforms to Industry Athletics & Movement, LLC.

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from activities.
THE FOLLOWING ENTITIES OR PERSONS: Industry Athletics & Movement, LLC and their employees, volunteers, or the activity or event holders, activity or event sponsors, activity or event volunteers.
I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise. The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old)
The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor, the parents, of the legal guardian.
SPRING SHOW POLICIES
I agree and understand that all students registered for Winter/Spring Session participate in our Annual Spring Show and are responsible for all fees and extra time commitments that are associated with the Spring Show.
I agree and understand that parents/guardians are required to pay the applicable participation and costume fees by their due dates. These fees are non-refundable and no transferable regardless of the students participation.
I agree and understand that students are required to attend 90% of their classes in order to perform in the show.
I agree and understand that students are required to participate in rehearsal week, dress Rehearsal, and the spring show.
I agree and understand that all communication is sent via email and occasionally by text, and it is the responsibility of the parent/guardian to read and retain all information shared by IAM and ensure their dancer is at all required rehearsals and performances on time, and prepared with all applicable costumes/equipment.
BY SIGNING THIS WAIVER, YOU AGREE UNDERSTAND AND AGREE TO ADHERE TO ALL OF THE POLICIES, RELEASES OF LIABILITY, AND WAIVERS IN THIS DOCUMENT.
