When you visit our Website, we do not collect any personally identifiable information without your consent. We collect and store information from you that you voluntarily choose to give us or enter on our website. When you register for classes at CMT we collect contact information such as parent/guardian names, addresses, phone numbers, e-mail address and student information such as names, birthdays, skill level, etc. This information is needed to manage studio business and will not be used or sold for any other purpose, except as described below.
CMT will access your personal information and/or share it with third parties only for the following purposes:
-as needed for studio business
-if required to do so by law or to comply with legal process
-to protect the rights, property, or personal safety of the public.
No bullying, harsh language, or any manner of disrespect towards another person is tolerated. At Broadway Bound, we value creating a welcoming environment for everybody that walks through our door! If such negative behavior is spotted, a teacher may mark down the student as a "strike" on their record with us.
1 Strike: The instructor/administration will speak with the student privately.
2 Strike: The instructor/administration will speak with the student's parent privately.
3 Strike: Administration will decide to either not let the student participate in the next production/show, or to cease their participation at Broadway Bound altogether.
All communications are made via the email and/or phone number you provided when creating your account. If you provide us with an incorrect email/phone number, you understand it is YOUR responsibility to edit it on the Parent Portal. We may adjust it for you if an email is made to Sarah Hamilton at email@example.com requesting to do so. However, you agree to not hold us liable for an absence of information due to you not providing updated contact information.
You recognize that by registering, you and/or your child(ren) are in the risk of being exposed to or being infected by COVID-19. By signing, you agree that you will not hold Broadway Bound responsible for being exposed to or receiving COVID-19.
Recently we have had several students arriving too early or being picked up too late. This is concerning to us, as we wish to provide the best safety as possible for your child. Our building is not open until approximately 5 minutes before each class. In which, students arriving half an hour early for example to classes is worrisome to their safety as they are unsupervised and alone.
We would also like to ask for students to be picked up no more than 5 minutes after their class ends, as our teachers are not able to watch and stay at the building with them.
Concerning these issues, we are enacting the following consequence for early drop off OR late pickup:
-Dropped off earlier than 10 minutes before class time OR picked up 10 minutes after class ends- $5.00 charged to your account
More than 10 minutes- Added-on fee of $1.00 for each minute.
-Example: If you were to drop off your child at 4:40 for their class at 5, you would be charged a total of $10.00 ($5 for 10 minutes and $5 for 5 minutes on top of that).
Thank you for understanding the dire need to keep your child safe and offer the best experience possible at Broadway Bound. All classes begin at their scheduled time & end at their scheduled time (no classes will be ending earlier than planned).

I registered my child to take a class at Broadway Bound WCCMT. I understand that this Participation Agreement (“Agreement”), together with the confirmation email and other waivers I received at time of registration, sets out the terms for enrollment. I understand that my signing it is part of the registration process and in consideration for my child’s participation in the class. This Agreement uses certain terms. It refers to my child as “Student” and to classes as “instruction.” It calls Broadway Bound WCCMT’s building the “Facility.” Participation in instruction, rehearsals, performances, and other Broadway Bound WCCMT events, whether in-person or in an online setting, and presence in or at the Facility, are together referred to as “Class Activities.”

I confirm that the information I provided in the registration form, including information about emergency contacts and medications, is true and correct.
CONSENT TO MEDICAL CARE
I authorize Broadway Bound WCCMT to provide Student first aid and to arrange medical assistance, transportation, and emergency medical services for Student if Student gets hurt while participating in activities at Broadway Bound WCCMT. I will be responsible for any costs related to Student's medical treatment and transport. I understand that Broadway Bound WCCMT does not provide health, medical, disability, or other insurance coverage for Student or for me.
I understand that Student’s participation in Class Activities has risks. Risks may be inherent, such as physical movement, exposure to noise, exposure to infectious disease through close contact with other people including people singing, projecting their voice, or other activities or environments. I understand that such risks may also arise from Student’s own actions, from Student leaving the Facility unaccompanied without permission, or from the actions of others at or near the Facility or encountered while traveling to or from the Facility. I understand that School cannot guarantee that Student will willingly participate in performances, or obtain employment in the performing arts or in any other field. With such information and awareness, and with the recognition that other factors may create additional such risks, I knowingly, freely, and voluntarily: (a) enroll Student in instruction at Broadway Bound WCCMT; (b) authorize Student to participate in Class Activities; and (c) assume and accept the risks of all physical and emotional injury, illness, exposure to disease, death, property damage or loss, financial obligation, loss of privacy, loss of reputation, and all other injuries and other consequences, whether known or unknown, whether foreseen or unforeseeable, and whether incurred at the Facility, Student’s residence, or other locations, that may result, directly or indirectly, from Student’s participation in Class Activities, regardless of the cause. Since some Class Activities involve physical activity, injuries may occur. Each student may decline to participate in any activity that may be harmful and is responsible for informing the instructor of any physical limitations which prevent full participation in class.
LIABILITY WAIVER & RELEASE
I waive and release Broadway Bound WCCMT and its directors, officers, agents, employees, volunteers, and affiliates (collectively,
“Staff”) from any and all liability, claims, costs, and expenses of any kind and of whatever nature which Student or Student’s heirs, next of kin, or legal representatives may have or which may later accrue, caused by or arising directly or indirectly from Student’s participation in Class Activities, including, without limitation, any exposure to infectious diseases, any injuries suffered while rehearsing or performing, any related medical care, or any travel to or from the Facility. This release and waiver includes, in each such case, all claims in respect of the risks noted above, known and unknown, foreseen and unforeseeable, regardless of the cause or whether such claims arise from tort, contract, or otherwise, and even if caused by negligence, whether passive or active. I will not sue any of the Staff on the basis of these waived and released claims. I waive and release all Staff from any and all liability, claims, costs, and expenses of any kind and of whatever nature, which I in my personal capacity may have or which may later accrue, caused by or arising directly or indirectly from Student’s participation in Class Activities, including, without limitation, my presence in the Facility, attendance at Broadway Bound WCCMT performances or other events, or interactions with Student, other students, or any Staff.
I confirm that I am signing this digital document on behalf of, and as an agent for, any other individual who may be a parent or guardian of Student, that I am fully authorized to do so, and that I am binding the individual, Student, and such other parent(s) or guardian(s). This Agreement will be legally binding not only on Student and me but also Student’s and my heirs, next of kin, and legal representatives, and will run in favor of, and may be enforced by, each of the Staff. It will be binding to the fullest extent permitted by law. If any provision of this Agreement is found to be illegal, invalid, or unenforceable, the remaining terms will be effective. This agreement will be governed by Utah law.
I have read this document and understand that by signing this Agreement, I am giving up legal rights and/or remedies that may be available to Student, to me and to any other parent or guardian of Student, and to their respective heirs, next of kin, and legal representatives.
Classes are charged monthly, according to how many classes will be held the following month (specific class rates depend on the class), with no refunds given for classes missed by the student. Charges are made on the 1st of every month, due by the 10th, with a $10.00 late charge added on the 15th. If a student wishes to drop their class(es), they must drop by the 10th of that month in order to not be charged tuition. If dropped after the 10th, the student still owes full tuition for that month.
In order to pay for our building and staff needs so that we can provide the highest quality of performing arts education, we have enacted the following: If payment is not made by the 15th, the student will be notified that they are not able to come to class until payment is made. If payment is still not made by the 1st of the next month, the student will be removed from class and cannot register for classes at Broadway Bound until they have a $0 balance.
If a student misses 3 times before a performance or if they register late (after classes begin), the teacher has the right to not let the student perform in the next performance. The student is very welcome to still attend class and participate in the next showcase/recital.
We unfortunately do not have a water fountain at this time and want your student to be healthy! By accepting this, you agree to always provide your student with a water bottle. We strongly recommend bringing a reusable water bottle to prevent waste and trash. Thank you!
