Aloha Style Payment Terms and Conditions
Drop-in Students
Payment is due at the time of sale for each class a student attends. Payments can be made by cash or check when class is held at the studio. Checks can also be mailed to Aloha Style Polynesian Dance 303 Crescent Hill Drive, Havertown, PA 19083. Credit card payments may be made via the Aloha Style Dance Studio-Pro Student Portal.

Seasonal/Annual Memberships
Monthly Payments
If a member wishes to make the monthly payments themselves, payments can be made by cash or check when class is held at the studio. Checks can also be mailed to Aloha Style Polynesian Dance 303 Crescent Hill Drive, Havertown, PA 19083. Credit card payments may be made via the student portal. Payments for the amount of the Seasonal or Annual Membership are expected to be paid in full by the end of the membership.

Automatic Monthly Draft
If a member wishes to be set up with Automatic Monthly Payments, payments will be drafted from your checking or savings account, debit or credit card (Visa, MasterCard, Discover or Amex) for the length of the membership.
Joiners Fee
A one-time joiners fee of $25 is due for all members upon joining Aloha Style Polynesian Dance.
Termination
This is a continuous membership plan. It is my understanding that if I wish to terminate or change my membership in any way, I must give the Aloha Style Polynesian Dance written notice. Cancellation will be effective after the next billing cycle and membership will continue for the next 30 days.

Authorization For Draft Payments
By clicking submit below, I have given authority to my Bank and/or Credit Card company to honor pre authorized payments drawn by Aloha Style Polynesian Dance on my designated account for membership payments as indicated above. It is understood that Aloya Style’s sending of a pre authorized payment to the Bank or Credit Card company as payment becomes due shall constitute valid notice of such payment due on this membership. When the Bank or Credit Card company honors the payment by charging my account, such payment shall constitute my receipt. Should any pre authorized payment not be honored by said Bank or Credit Card company when received by them, then it is understood that payment is to be made by me in the amount of said payment.
Returned Payments
Should my Bank or Credit Card Company not honor any membership draft for any reason, I realize that I am responsible for that payment plus a $15 SERVICE CHARGE applied by Aloha Style Polynesian Dance and any service fee my Bank or Credit Card Company may make.
Discrepancies
I understand that all billing discrepancies must be reported within 90 DAYS of the original transaction date. Automatic drafts will be withdrawn on the business day closest to my assigned draft date.
RELEASES & WAIVERS OF LIABILITY
I understand that Aloha Style Polynesian Dance assumes no responsibility for injuries or illnesses which I, or those on my membership, may sustain as a result of physical condition or which may occur upon the premises of Aloha Style Polynesian Dance prior to, during, while participating in, or subsequent to any Aloha Style Polynesian Dance activity, and hereby release and discharge Aloha Style Polynesian Dance from any and all claims for injury, illness, death, loss or damage which I may suffer as a result of participation in these activities.


I fully understand that physical exercise has been associated with certain risks. Aloha Style Polynesian Dance recommends consulting with your physician before beginning an exercise program.

I acknowledge that Aloha Style does not carry an accident or health insurance policy on a member at any time.


In an emergency, I authorize Aloha Style to inform my emergency contact and if necessary, to obtain emergency medical treatment for any person listed on this membership. I accept responsibility for all fees incurred in the care and transportation by ambulance to the Hospital Emergency Room.
I understand that Aloha Style is not responsible, nor liable for personal property lost or stolen while I am participating in events/classes for Aloha Style. I hereby discharge, release and waive Aloha Style from any and all irresponsibility in connection therewith.
I give my permission to Aloha Style Polynesian Dance to use, without limitation or obligation, any photographs, videos, and/or tape recordings of which may include my image or voice or that of others included on my membership for promotion of Aloha Style Polynesian Dance, UNLESS I NOTIFY ALOHA STYLE IN WRITING PRIOR TO PARTICIPATION.

I hereby release the directors/owners/landlords of Aloha Style Polynesian Dance and its affiliates from any and all claims for damages or injuries which I may sustain while participating in any activity connected with them or upon the premises. I will not hold the school or its affiliates responsible for articles lost or stolen.
I, intending to be legally bound, hereby consent to the unrestricted use by Aloha Style Polynesian Dance, its successors, heirs, and assigns, of my name; portrait; statements made by or attributed to me; photos mechanically reproduced, for use by said Aloha Style Polynesian Dance in newspaper, magazine, booklets, folders, or other advertising media covered by the advertising program. Furthermore, I authorize that Aloha Style Polynesian Dance may, in their discretion, or in the discretion of any of them, use, sell, distribute, publish and republish the same, either separately or together with my name or a fictitious name.
