I understand that Kristene Watson DBA On Stage Dance Supply, and her employees are not responsible for any injuries sustained prior to the beginning of lessons, during the lesson itself or afterward. I recognize that my child’s participation may expose him/her to the risk of injury or harm. I accept the risk and hereby release Kristene Watson DBA On Stage Dance Supply, its agents and employees from all liability for illness and/or personal/property damage occurring during instruction or performance. I understand that the instructor is not responsible for any injury, illness or property damage occurring during instruction or performance. I certify that my child is in good health and capable of participating in all of the activities.

I am releasing my student’s photo to be used for promotional purposes associated with On Stage Dance Studio, including Facebook and other social media that represents On Stage Dance Studio, as well as posters, fliers, and other promotional materials that are released from On Stage Dance Studio.

I understand that even if my student is not present in class, I am still responsible for the full tuition amount.

I understand that my student may risk not being able to perform in recital with NO compensation for any payments made toward costume, tuition, or any other goods/services if:
• The student is absent more than six times in 1 semester.
• The student is absent more than two consecutive practices.
• The student is absent to dress and tech. rehearsals without letting the instructor know.

I acknowledge that it is my responsibility to let the instructor AND Kristene Watson know if my student withdraws from class. If I fail to communicate this information, I will be responsible for tuition payments even though my student is not present in class.

I understand that if my child withdraws from class and does not perform in the recital, I forfeit the right to the costume and will receive NO compensation for any other fees paid pertaining to recital.

I understand that if a make-up lesson is scheduled, attendance will be taken and counted just as a regular practice time.

I understand that if my tuition payment is late (after the 10th of each month), a late fee of $15 will be added to the tuition charge each month that this occurs.

I agree to pay my account balance in full before my student can receive his/her costume.

I have read, understood and agree to be bound by the above statement as the parent and/or guardian of the student who is enrolled at On Stage Dance Studio, Tahlequah, OK.
