I understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of dance. In recognition of the acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including all claims of negligence arising as a result of such activity from which liability could accrue to Jennifer Leigh’s Dance Gallery, its’ members, officers, agents, employees, instructors, assistants, subsidiaries, and all affiliated entities (hereinafter collectively referred to as “Jennifer Leigh’s Dance Galleryâ€.
The risks of injury and illness (the term illness includes communicable diseases such as MRSA, influenza, and Covid-19) to my child from the activities involved in the aforementioned programs are significant, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury or illness does exist and:
1. For myself/others, and my child, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of others, and assume full responsibility for my child's participation; and,
2. I willingly agreed to comply with the programs stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child's readiness for participation and/or in the program itself, I will remove my child from participation and bring such attention of the nearest instructor or staff member immediately; and,
3. I, the parent/guardian, assert that I have explained to my child: the risks of activity, her/his responsibilities for adhering to the rules and regulations, and that my child understands this agreement.
I hereby agree to release Jennifer Leigh’s Dance Gallery and hold Jennifer Leigh’s Dance Gallery harmless of all liability, and hereby acknowledge and I knowingly assume full responsibility for all risks of physical injury arising out of active participation in dance classes on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
The participant has my permission to participate in Jennifer Leigh’s Dance Gallery classes, events, and activities. I warrant that the below information is complete and correct. I further release Jennifer Leigh’s Dance Gallery of all liabilities associated with my child’s attendance at Jennifer Leigh’s Dance Gallery.
If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights. By checking the box below I agree to the above terms & conditions.
