I, the parent or guardian, hereby gives permission for any and all medical attention to be administered to my child, in the event of accident, injury, sickness, etc., under the direction of the physician listed on file or at any necessary emergency facility, until such time as I may be contacted. I also assume the responsibility for the payment of any such treatment. This release is effective for the period of one year from the start of classes registered.

I give full rights to Studio 48 and its staff to use any photography and videography taken of me or my child during Studio 48 activities for studio purposes. Photos and video may be used in brochures, newspapers, websites, advertisements, social media and other promotional material created for Studio 48. Promotional materials may appear with or without names. I have read, understand and agree to the above stated waiver of liability, medical and photo releases. I have read, understand, and agree to all policies and expectations as dictated in Studio 48's “Studio Policy Handbook”. I understand I will be held responsible for all fees, tuition, costume payments, and late fees as outlined by Studio 48.

I hereby give my child permission to partake in activities at Studio 48. I waive the right to any legal action against Studio 48 for any injury sustained on studio property or at any Studio 48 event. I understand that I am enrolling my child in a program that may involve physical activity and have agreed that my child is in good physical condition and does not suffer from any disability that would prevent or limit participation in physical activities organized by Studio 48.
