I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury and, in rare circumstances, death. I agree to release and hold harmless Elevate Dance Academy, including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Elevate Dance Academy liable for any personal injury or any personal property damage or loss, which may occur on or off the premises before, during or after classes. Furthermore, I understand that I should be aware of my, and/or my child’s physical limitations and agree not to exceed them. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights. Further, I authorize and agree that Elevate Dance Academy may take and use photographs/videos of me and/or my child for purposes of record-keeping, advertising, and marketing. I understand that I do not have any rights to these photographs/videos and will not be compensated for the same.

I have received or will obtain a copy of Elevate Dance Academy's policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Elevate Dance Academy does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. If a class is dropped, we require 30 days notice and will charge the next round of tuition if it falls within the 30-day window. Registration fees are non-refundable.

Policies and Procedure Waiver January 2023

Thank for your choosing to be part of Elevate Arts Academy. Please review the following policies and procedures and sign below.

1.) Attendance
Attendance is vital to the student's success. Please attend all classes, arrive five minutes before class, and wear the proper attire as outlined by instructor or company director. No make-up classes will be given for no-show classes. If an absence is reported prior to the missed class, a make up can be arranged. No tuition credit will be given for no show classes. Please communicate with us about extenuating circumstances.

2.) Tuition and fees
Tuition will be charged on the 15th of each month, for the following month's tuition. If you need to drop a class, you must inform Elevate through email or phone before the 15th of the month. No refunds will be given after tuition is run, however we can provide partial credit your studio account for future classes if a drop needs to occur after the tuition for the following month has been charged. All recreation students who choose to participate in the end of year recital will be charged a $65 costume fee (per class performing in the recital) and a one time recital fee of $35 for an individual OR $50 for a family of more than one student performing.
***Competition company teams have a separate system for fees.

3.) Recital
We encourage all students to participate in the end of year recital. This is a culminating event to showcase your student's hard work during the year. We perform at beautiful facilities, create a lovely program of dances, and love to see the students perform. You may opt out of the recital if you do not desire to participate. Please let us know as soon as possible. No refund for costumes or recital fees will be given if we are not made away of your choice to opt out of the recital.

4.) Injury
We employ the best instructors who have danced the majority of their lives and many of whom have studied performing arts on a collegiate and/or professional level. We provide a clean, safe facility, with constant adult supervision. Injury's do happen. By signing this waiver you are accepting the risk that your student may sustain an injury during his/her time in class. By signing this waiver you are are accepting full responsibility for your student’s potential injury and will not seek retribution from instructors, staff or ownership of Elevate Arts Academy. We do our absolute best to challenge and push our students within reason and safe practices. Your students’ safely and health is more important than anything else. Please talk to us if you ever have the slightest concern.

5.) Communication
Elevate uses email as our primary form of communication. We send reminders about tuition balances, class events, recital information, studio closures and other important updates. Please do your part to be receptive to our emails. "Star" them if needed, so you don't miss anything important.
