You must be over 18 years of age to register for an account.
Student/Account holder must read and accept all terms for registration:
- Tuition for our Fall/Spring season are due on the 1st of each month. A late fee of $15.00 will be assessed to all accounts paid after the 10th of the month.
- Any student who wishes to withdraw from a class must submit a completed "SOC Class Withdrawal Form" to the front office in person or via email (to email@example.com) by the 20th of the month, or the following month's tuition will be charged.
- Tuition for our Summer camps and classes are due in full upon registration.
- Private lessons must be cancelled/rescheduled no later than 24 hours prior to lesson time or the full session fee will be charged.
- There are no refunds for missed classes. Makeup classes are not available at this time due to class size limitations.
- All returned NSF checks will be assessed a $35.00 fee.
- All registration and tuition payments are non-refundable.
- If conditions require in person classes to be temporarily suspended, all classes will be conducted online via Zoom until conditions allow classes to resume in person.
- All Students/Guardians are required to sign and review our SOC Studio COVID-19 Waiver Liability form before attending classes and/or events at Spark of Creation Studio.
I for myself and/or my minor child release, discharge and agree to hold harmless Spark of Creation Studio its contractors, staff and agents for any liability claims or demands for personal injuries, sickness or death as well as for expenses due to loss or damage to personal property or lost wages. I understand that dance is a physical activity and injuries may occur. If an injury/accident should occur, and I cannot be notified, I authorize Spark of Creation Studio to seek emergency treatment for my child at my own expense. I also grant permission to use videos/photos taken of the above student(s) for advertising or promotional purposes. I assume full financial responsibility for all tuition and fees and agree to abide by all studio & payment policies as outlined here and in the studio handbook.
