You must be over 18 years of age to register for an account.

Student/Account holder must read and accept all terms for registration:

- Tuition for our Fall/Spring season are due on the 1st of each month. A late fee of $15.00 will be assessed to all accounts paid after the 10th of the month.

- Any student who wishes to withdraw from a class must submit a completed "SOC Class Withdrawal Form" to the front office in person or via email (to admin@socstudio.com) by the 20th of the month, or the following month's tuition will be charged.

- Tuition for our Summer camps and classes are due in full upon registration. There are no refunds for summer camps and classes.

- Private lessons must be cancelled/rescheduled no later than 24 hours prior to lesson time or the full session fee will be charged.

- There are no refunds for missed classes. Makeup classes are not available at this time due to class size limitations.

- All returned NSF checks will be assessed a $35.00 fee.

- All registration and tuition payments are non-refundable.

- All Students/Guardians are required to sign and review our SOC Studio COVID-19 Waiver Liability form before attending classes and/or events at Spark of Creation Studio.

I for myself and/or my minor child release, discharge and agree to hold harmless Spark of Creation Studio its contractors, staff and agents for any liability claims or demands for personal injuries, sickness or death as well as for expenses due to loss or damage to personal property or lost wages. I understand that dance is a physical activity and injuries may occur. If an injury/accident should occur, and I cannot be notified, I authorize Spark of Creation Studio to seek emergency treatment for my child at my own expense. I also grant permission to use videos/photos taken of the above student(s) for advertising or promotional purposes. I assume full financial responsibility for all tuition and fees and agree to abide by all studio & payment policies as outlined here and in the studio handbook.

- All accounts need to register for our auto-pay system.
- Any student who wishes to withdraw from a class shall request the form from the front desk and submit a completed "SOC Class Withdrawal Form" by the 20th of the month, or the following month's tuition shall be charged.

SUMMER SESSION CANCELLATION POLICY:
- Please understand that there are a very limited number of spaces in our summer camps and classes. For this reason, all summer camp and/or class tuition fees shall be non-refundable.
- Any student/parent requesting to cancel their enrollment in a summer camp or class shall submit their request via email to admin@socstudio.com at least 30 days prior to the start of the camp or class. All camp or class cancellation requests received at least 30 days prior to the start of the camp or class shall have the option of transferring to an equivalent camp or class (if space permits) or having their full camp or class payment credited on their account which can be used toward future camps or classes.
- All camp or class cancellation requests received with less than 30 days notice prior to the start of the scheduled camp or class shall have the option to transfer to an equivalent camp or class (if space permits) or receive a 50% credit of the paid camp or class tuition which may be used towards future camps or classes. The student/parent would lose the remaining 50% of their tuition payment.
- All charges through our DSP merchant services will apply a 3% processing fee.

ALL SESSIONS:

Assumption of the Risk and Waiver of Liability
Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Spark of Creation Studio (“SOC Studio”) has put in place preventative measures to reduce the spread of COVID-19; however, SOC Studio cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending SOC Studio could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending SOC Studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at SOC Studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, SOC Studio employees, volunteers and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at SOC Studio or participation in SOC Studio programming (“Claims”). On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless SOC Studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of SOC studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during or after participation in any SOC studio program.
