The student/participant named above, does voluntarily participate in any and all Royal Performing Arts Center (RPAC) activities. The above student and I understand that dance is an activity with inherent dangers for injury from participation and involvement with Royal Performing Arts Center classes and in its various formal and informal activities. These activities include but are not limited to dance and dance performance, acrobatic dance classes and fitness classes. Royal Performing Arts Center is not responsible for any lost or stolen property, at any time. Anyone found to be in violation of rules, codes of conduct, or found to be disruptive (vocally or actively) to either another individual or group, may be asked to leave the premises or off-site location at any time and be refused reentrance without refund (full or partial). Registration is non-refundable (Covid-19 exception-refunds will be issued if classes can not be held due to Covid-19 state regulations). Tuition, costumes, competition, and travel fees are non-refundable. Credits and/or refunds are not issued for class absences. Students are enrolled, unless otherwise agreed upon, for the entire dance season the student is registered for. Recital performances are optional. Individual, group and class photos/videos/sound bytes taken by the studio, or any professional company hired by the studio or event (such as competitions), may be used in any or all forms of media advertising. Rules and codes of conduct will be posted both on the studio website and at the studio.
It is understood that dance is an activity with inherent risks for injury. Our staff is professionally trained and will train students in the safest manner possible. The studio does NOT carry medical insurance for its students. It is required that all students be covered by their own policy; and, if an injury occurs, it is understood that the family policy will be the only source of reimbursement/coverage. I hereby release and hold harmless, Royal Performing Arts Center, it's staff and all agents acting on behalf of RPAC, from all liabilities, suits and claims of any kind, including legal or financial, arising from activities either by participation and/or observation at any Royal Performing Arts Center activity.
I authorize Royal Performing Arts Center, it's staff and agents acting on behalf of RPAC, to use their best judgement in providing or arranging medical care in the case of an emergency.
