WAIVER AND RELEASE OF LIABILITY
I acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, which might result not only from their own actions, inactions or negligence, but the action(s), inaction(s), or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time. I assume all foregoing risk and accept personal responsibility for the damages following such injury, permanent disability or death.
I agree to release, waive, discharge and covenant not to sue The Dance Company along with their respective directors, instructors, guest choreographers, participants, and, if applicable, owners and leavers of premises used to conduct the event, all of which are hereinafter referred to as “releasesâ€, from any and all liability to each undersigned, his or hers next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the release of otherwise.
TUITION AGREEMENT
I accept full financial responsibility for all related costs during the 2022 - 2023 dance season at The Dance Company. These costs include registration fees, tuition, costume payments, and late fees (if applicable). I understand that full payment is required on the 5th of each month. I also understand that if my account becomes delinquent, the student will not be able to participate until the account is current. I understand my tuition payment is based on a yearly tuition, which accounts for the number of days the student attends. If a student misses a day due to personal reasons, no refunds will be given. If a student misses a class due to inclement weather or teacher absence, The Dance Company will offer a comparable makeup class.
