Thank you for your continued support and trust in Centre Stage. While attending any class at Centre Stage LLC there is always the possibility of injury. Centre Stage will provide responsible care for my student, but, if an injury should occur, I, the undersigned, agree not to hold Centre Stage LLC, its owners, chaperones or any of its staff responsible. I further agree to indemnify and hold harmless Centre Stage LLC and its agents and employees from claim, losses, injuries, and damages of any nature whatsoever incurred. I further authorize the agents and employees of Centre Stage LLC to obtain any and all necessary emergency health care for me. Parents/Caregivers will be the first people notified immediately should an injury more severe than a minor cut or fall occur. Please use your utmost common sense with regards to your own safety and children’s safety when inside the studio premises. *INITIAL ___

As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as the “Coronavirus”, at any time or in any place. Be assured that we have always followed state and federal regulations to limit transmission of all diseases in our office and continue to do so. Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of Coronavirus. Although we have taken several measures to provide social distancing and contactless participation, please know that participation in Centre Stage instructional classes is at your own risk. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin HEREBY RELEASE AND HOLD HARMLESS Centre Stage, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Tuition is always due the first class of the month in which it has been assessed according to our Tuition Policy detailed on our website. If tuition is not received by the 10th of the month, you will be assessed a $10 late fee. If tuition is not received by the 20th of the month, another $10 late fee will be assessed. If tuition is a month or more late, the student will be removed from the roster and not allowed to return to class.

While attending class at Centre Stage, there may be a possibility of injury. Centre Stage will provide responsible care for my child (icepack, bandaid), but, if an injury should occur, I, the parent or caregiver of the child agree to not hold the Centre Stage, owner, teachers or staff responsible. In the event of an injury requiring medical attention, the parent will be the first to be notified for further instruction. I further agree to indemnify and hold harmless Centre Stage and its agents and employees from claim, losses, injuries, and damages of any nature whatsoever incurred.

We use social media to promote our business. We use Facebook and Instagram primarily. Signing this waiver allows us to post pictures of students on our social media accounts. We never list names or class names even with this waiver and we only post after the children have left the studio. We also monitor followers on our social media to ensure safety. We block accounts that look suspicious to us. Safety of our students is our first priority.
