I (we) hereby authorize Phoenix Dance Cooperative (PDC) to make charges to my Credit Card, Debit Card, or Bank Account on file within my DanceStudioPro PDC Parent Portal. This authority will remain in effect until Phoenix Dance Cooperative (PDC) is notified by me (us) in writing to cancel my dancer’s membership or until the end of the 2024/2025 dance year. I understand that the DSP Merchant Services convenience fee will be applied to all transactions except monthly tuition at the rate of 3.05% + .30¢ for credit card payments and 1% + .20¢ for electronic bank transfer (e.g. checking account). The online portal provides the option for me to select either form of payment and to change it at any time.

RELEASE FROM LIABILITY I agree and represent that all exercises, treatment and use of all PDC facilities shall be undertaken at my own risk, that I am in good physical condition and physically able to undertake any and all physical exercises and treatments provided by PDC, and that PDC and/or affiliated companies and/or their respective agents and employees, shall not be liable for any claims, demands, injuries, damages, actions or causes of action, whatsoever, to me or my property arising out of, or connected with the use of the services and/or facilities of such corporation and of any affiliated companies and/or their respective agents and employees or the premises where the same are located, and I do hereby expressly forever release and discharge said corporation and any affiliated companies and their respective agents and employees, from all such claims, injuries, damaged, actions, or causes of action.

I grant to Phoenix Dance Cooperative the right to take photographs and/or video of my child in connection with PDC dance, classes, recital and other events related to Phoenix Dance Cooperative. I authorize Phoenix Dance Cooperative, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Phoenix Dance Cooperative may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, Facebook, Instagram and Web content.
