COVID Release

The novel coronavirus, COVID-19, has been declared a world-wide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact.

Symptoms of COVID-19 include, but are not limited to: Fever or Chills, Cough, Shortness of Breath or Difficulty Breathing, Fatigue, Muscle or Body Aches, Headache, Loss of Taste or Smell (not normally present), Sore Throat, Congestion or Runny Nose, Nausea or Vomiting, Diarrhea. As a result, federal, state, and local governments and health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

The Company Space has put in place preventative measures to reduce the spread of COVID-19, however, The Company Space cannot guarantee that you or your children will not become infected with COVID-19. Further, attending The Company Space could increase your risk and your child(ren)'s risk of contracting COVID-19.

By signing the agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren)'s risk of becoming exposed to or infected by COVID-19 by attending The Company Space and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at The Company Space may result from the actions, omissions, or negligence of myself and others, including but not limited to, studio employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including but not limited to, personal injury, disability, and death)., illness, damage, loss, claims, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at The Company Space or participation in studio programming. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless The Company Space, its employees, agents, and representatives, of and from "Claims", including all liabilities, actions, damages, costs or expenses of any kind arising out of or relating thereto, I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of The Company Space, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during or after participation in any Studio program.

In-Studio Classes

Students who wish to attend in-studio classes must complete a health questionnaire and will be screened for symptoms of COVID-19. Students will be required to comply with The Company Space Health & Safety Protocol.

Flu-Like Symptoms: Any student (or their immediate family member) experiencing flu-like symptoms, we ask that you please not attend in-studio classes until you have been medically cleared. We will work with students on make-up classes for anyone who can't participate due to illness.

Face Coverings: Face coverings are required for all individuals while outside of the dance rooms.

Temperature Checks: The Company Space Teachers & Staff will receive temperature checks every day. Students will have their temperatures checked before entering the building.

Limiting Dancer Capacity: The Company Space is enforcing the 6-foot social distancing in all classrooms; therefore, we will not be allowing drop-ins, or make-ups in our studio classes. (Zoom classes will continue to be available and can accommodate make-up lessons.) This will help to give dancers the optimal amount of space to continue practicing social distancing.

Class Schedules: We will also be scheduling classes without breaks and limiting the total number of hours students will be "in-studio" to reduce the need for meal breaks. Students should eat prior to coming to the studio and only bring water and a small energy snack if necessary.

Class Observers: For the safety of all, The Company Space will not allow visitors, parents or friends to observe classes at this time.

Dressing Rooms: Our dressing rooms will be closed to help comply with social distancing. Student dress code will be adjusted to the following:
Dancers will be required to remove their street shoes at the entrance of the building and stored in cubbies. (We encourage students to wear flip flops for quick transitions.)

Dancers will be required to bring a laundry basket (or other bag), water bottle, yoga mat, personal hand sanitizer and their dance shoes. (Label all items with your name and identify your laundry basket with a ribbon or some other way to recognize it quickly.)

Hand Washing: All students and faculty should wash their hands multiple times throughout the day. The CDC recommends this as one of the best practices to avoid transmission of the virus.

Studio Cleanliness: Each studio room, as well as our restrooms and common areas, will have increased cleaning of all surfaces, handrails, floors, etc. Ballet students will be asked to use a disposable cleaning wipe to clean their ballet bar prior to use.

The Company Space will ask you to provide information that personally identifies you, your location or your contact information when you enroll in dance classes or purchase dancewear, request information from us, or register on our site for any other reason. We will use your personal information to fulfill your specific request and also to make contact with you related to that request. For example, when you provide personal information to enroll in a dance class, we will use that information to register the student, but we will also use that information to contact you about billing and service matters or if we have a question or dispute with you. If you contact us with questions about our products or services, we will use your contact information to answer your specific questions, but may also use your personal contact information to provide additional materials about our products at a later date.

The Company Space will access your personal information and/or share it with third parties only for the following purposes: (i) as needed to confirm compliance with the terms of your registration agreement or to enforce our rights under a registration agreement; (ii) if required to do so by law or to comply with legal process; (iii) to protect the rights, property, or personal safety of the public; or (iv) in connection with a merger, consolidation or sale of substantially all assets of The Company Space. The Company Space will not sell your personal information to anyone, except that The Company Space may sell all rights to some or all of our products and the sale of your personal information will be part of that sale.

The Company Space is committed to ensuring the security of your personal information. The Company Space stores your personal information in secure operating environments that are not accessible by the general public. Credit card numbers are used only for processing payment. Credit card information is accepted only through a portion of our website that encrypts the credit card information before transmission for your added protection.

The Company Space also gathers and uses data that includes some of your personal information, but in a way that removes all of the parts of the personal information that could be used to identify you. That information is used for analyzing the effectiveness of various marketing strategies and also for analyzing the performance and use of our products. We may share this collected information for marketing or product improvements.

Cookies are pieces of information that a website transfers to your computer. Our products and websites use only “per session” cookies. These cookies do not collect information from your computer, they do not personally identify you, and they are completely erased when you close your web browser. They are used to maintain session identification while you are logged into the website. Most browsers allow users to refuse cookies. Please refer to the information about your browser. You must accept “per session” cookies to use our software.

The Company Space is committed to complying with the requirements of the CAN-SPAM Act of 2003. The Company Space is taking care and has implemented commercially reasonable practices and procedures designed to effectively prevent sending a commercial electronic mail messages to anyone who has let us know that they do not want to receive those messages.

Social Media/Video/Photography Release

I do hereby consent to the photographing of my child and the recording of his/her voice and the use of these photographs and/or recordings singularly or in conjunction with other photographs and/or recordings for advertising, publicity, commercial or other business purposes. I understand that the term "photograph" as used here encompasses both still photographs and motion picture footage. I hereby release The The Company Space and any of its associated or affiliated companies, their directors, officers, agents, employees and customers and appointed advertising agencies, their directors, officers, agents, and employees from all claims from every kind on account of such use.

As the legal parent or guardian, I release and hold harmless The Company Space, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of The Company Space, its owners and operators or in route to or from any of said premises.

The studio reserves the right to suspend or dismiss any student whose attitude, attendance or conduct is found to be unsatisfactory. In such a case a refund will be authorized on a case by case basis.

The undersigned gives permission to The Company Space, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian, or a medical emergency requires immediate action. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

I certify that I am responsible for all payments and fees.

Registration Fee

There is a $25 non-refundable Registration Fee (sibling discounts apply). Each additional sibling will be $10.

There is a two month minimum for all classes. The month of September is non-refundable. One-month notice from the first of the month is required to discontinue any classes at The Company Space. Request to Withdraw must be received at the studio office. Withdrawal must be done with the studio office and not with the teacher. Withdrawal must occur within the first 7 days of the month. No withdrawals will be accepted after April 1. To withdraw from classes a parent or adult student must inform studio administration in writing within the time period stated above. All automatic bank debiting or credit card charges will stop after the one-month notice period.

If you feel a discrepancy has been made on your account, you have 60 days to contact us to look into it for you. Beyond 60 days, no adjustments will be made.
