REGISTRATION POLICY:
In adherence with C.B. Dance Academy's (CBDA) Privacy Policy, I give my written consent to CBDA to use, without liability, pre-identified personal information of myself, and/or the student named above in order for CBDA to provide me or the above student with dance / performance / costume / registration services - including by email.

Students and parents automatically grant permission to CBDA and its Directors to use, without any compensation whatsoever, their photos and videos for publicity and promotional purposes, including but not limited to ads, television, videotaping and broadcasting, social media, web or print. I also agree to pay CBDA, upon registration, my child's dance fees for the full dance season, non-refundable annual Administration Fee, and a non-refundable Costume Deposit (for each recreational class and/or competitive class). I am aware that costumes will be automatically ordered and billed for each child registered. I also agree to pay the $35.00 Service Charge for each NSF / returned cheque.

I recognize that a risk of injury or potential health risks may be involved in participation in the programs / activities in which I am registering my child. I hereby willingly assume full responsibility for such risk of injury or health risks however caused for myself or for the above-named person for whom I am by law responsible during and after my / their participation in the program(s). In consideration of CBDA accepting this application, I hereby remise, release and forever discharge CBDA, its directors, instructors, officers, administrators, successors and/or volunteers participating in or connected with CBDA of and from all manner of action, causes of action, claims or demands against CBDA, I never had, now have, or may hereafter have, for or by reason of any loss, damage or injury sustained by me or the above named person from any medical therapy treatment advised or performed, or of the loss of any equipment used by me or the above named person, whether caused by the negligence of the releasee or otherwise. In the event, that my child requires physical assistance during the COVID 19 restrictions, I waive the 6 feet social distancing Government recommendation.

I acknowledge and agree to all the above.

I have been provided with a copies of the CBDA's "Policy & Guidelines and Privacy Policy" and have read them thoroughly prior to electronically signing this form and and agree to go through these guidelines with my child so that he/she is aware prior to class participation. I have also read and voluntarily agree to the CBDA COVID-19 and this Release and Waiver of liability / indemnity Agreement, and further agree that no oral representations, statements or inducements apart from the foregoing written agreement have been made.

C.B. Dance Academy (CBDA) has put in place preventative measures to reduce the spread of COVID-19 and variants;

Although CBDA is taking all the steps necessary to provide a COVID free environment, I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at CBDA or participation in CBDA classes. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless CBDA, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of CBDA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any CBDA program, class or event.

BY REGISTERING AND PAYING FOR ANY CLASS, EVENT, OR OTHER ACTIVITY THROUGH CBDA, YOU ARE AGREEING TO ALL OF THE ABOVE CONDITIONS AND WAIVERS. YOU ACCEPT AND AGREE TO ALL OF THE TERMS IN THIS AGREEMENT.

NO PEANUTS OR NUT PRODUCTS (including peanut butter) ALLOWED IN STUDIO.

Students are to arrive no earlier than 10 minutes before their class and are to be picked up no later than 10 minutes after their class. Younger students arriving early must be supervised. Teachers and CB Dance Academy staff are not responsible for students before classes commence or after they have finished.

Students should arrive dressed according to the dress code with all necessary hair accessories and shoes. Hair is to be worn back off the face for all classes. Ballet students are required to wear their hair in a bun. CBDA does not assume responsibility for lost, stolen or misplaced items. Please notify the studio in advance of a class absence.

Free Trial classes are offered to all students SUBJECT TO SPACE and CLASS AVAILABILITY in the chosen class.

Students are expected to act appropriately during class and not disrupt their teachers or other students. Parents will be notified if their child is misbehaving or causing a disruption. Upon a third notification, the child will be withdrawn from their class at the end of the current term.

If a student needs to be excused during class for any reason, they must ask permission from their teacher. No child will be released of their own accord. For early dismissal, parents must make prior arrangements and come to the door to pick up their child. This is a matter of safety.

Missed classes will not be made up and no refunds shall be issued. Cancelled classes (outside of Statutory Holidays) will be rescheduled accordingly.

Once registered, student class placement is considered reserved for the full season, and not just for a session. Instalment 1 fees are NON-REFUNDABLE.  Fees will NOT be refunded once a session begins.  In the case of a withdrawal from CB Dance Academy, the Director must be notified in writing immediately.  Until such time, the space will be held for the student and charges invoiced to the parents or guardians accordingly.  Upon withdrawal, fees will be recalculated at a per class rate, and the standard Withdrawal Fee of $45.00+HST per class will be processed to the account / credit card on file. All costume deposits and registration fees are NON-REFUNDABLE, NON-TRANSFERABLE. All second and third instalment class fees, comp fees and workshop fees associated with competitive students are NON-REFUNDABLE NON-TRANSFERABLE.

Parents must make an appointment to speak with a teacher or Artistic Director. Out of respect for students who are waiting for their class to begin, we ask that parents and students do not approach teaching staff between classes.

Gum chewing is not permitted in the classroom or reception areas. Food is not permitted in the change rooms. Beverages are not permitted in any studio room at any time (except for clear bottled water). Homework and eating areas are provided.

Students and siblings must always be supervised.

Students who deface or damage studio property will be charged for the cleaning, replacement or repair of such items.

All tuition is to be paid on the first of each month/term or in accordance with the terms in our fee schedule. Outstanding fees will be assessed a $45.00+HST penalty.  All NSF cheques will be assessed a $35.00+HST penalty.

All deposits are non-refundable / non-transferrable.

Students will be measured for costumes starting November 1st, and costumes are ordered at the end of the holiday season. Any students requiring a special order or size may be subject to additional charges. Costumes must be purchased to participate in the year-end recital. Costumes do not include required tights or shoes. Parents will be notified of any costume accessories required and not included with costume. All students will perform in the CBDA year-end recital - unless Parent advises CBDA in writing at time of Registration.

All accounts must be up-to-date and in good standing in order that year-end recital tickets and costumes are made available.
