I give permission to include photographs of my child in promotional material published by Grace Academy of Dance and Performing Arts. This could include, but is not limited to, newsletters, advertisements, website, and social media. Photographs will be for the purposes of illustrating dance activities at Grace Academy. Names will not be included without obtaining additional permission.
(NOTE: Parents who prefer not to have photographs used may opt out of this waiver within their parent portal)

I have carefully read and agree to the COVID-19 Safety Policies and Procedures set out by Grace Academy of Dance and Performing Arts. I acknowledge and am fully aware that Covid-19 has been declared a worldwide pandemic by the World Health Organization and is an extremely contagious virus. While Grace Academy of Dance and Performing Arts has put preventative measures into place to reduce the spread of Covid-19, they cannot guarantee that participants will not become infected. Further, participation in activities at Grace Academy of Dance and Performing Arts could increase the participant’s risk of contracting Covid-19.
I voluntarily agree to assume all foregoing risks and accept sole responsibility for an injury to my child(ren) or myself (including, but not limited to personal injury, illness, exposure to infectious disease or disability). I release and hold harmless Heidi Knapp, Grace Academy of Dance and Performing Arts, and teachers of the Academy from any and all liabilities, claims, actions, damages, costs or expenses arising out of or relating thereto.
I give permission to Grace Academy of Dance and Performing Arts and its teachers to seek medical treatment for the participant in the event that they are unable to reach a parent/guardian or emergency contact.

I have carefully read the fee policies of Grace Academy of Dance and Performing Arts and agree to pay fees owing in full by August 28th or upon registration (Autumn Term), and on or before the first day of each term thereafter. I understand that a late fee of $30 per dancer will apply if fees are more than 7 days overdue. I understand that no refunds will be provided once a new term has begun. I understand that classes may be a combination of in-studio and virtual at-home learning, and that in the case of an emergency or government mandated closure, classes will move to virtual learning only. No refunds or discounts will apply.
