IN CONSIDERATION of the enrolment of my child aforementioned above,

as a student of dance, the undersigned does hereby for him/herself, his/hers heirs, executors, administrators and assigns hereby waive any claims, demands or causes of action to which I or my child aforesaid may hereafter have or become entitled to by reason of personal injury occurring to my child while attending or participating in lessons or recitals and I do hereby release THRIVE DANCE ACADEMY, its representatives, agents and employees together with any person assisting on a voluntary basis in the conduct of dance classes in the studio and recitals or performances/activities outside the studio from any and all claims for personal injury so sustained.
I HAVE BEEN INFORMED and am aware that the dance classes in which my child is enrolled are an athletic activity and there are inherent risks of physical injury. To the best of my knowledge and information my child is not subject to any physical disability, medical condition, inherent weakness or disease which would in any way increase the potential for physical injury and I hereby undertake to advise THRIVE DANCE ACADEMY of any change in my knowledge or information in this regard.
I AGREE TO INDEMNIFY and save harmless THRIVE DANCE ACADEMY, its servants, agents and employees from any claims or demands which might be made against them by anyone acting on behalf of or in the name of my child or by my child upon the attainder of his/hers majority in consequence of personal injury sustained by my child while attending at or participating in dance lessons or recitals.
Photos/videos taken at class/competitions/recitals become property of THRIVE DANCE ACADEMY and may be used for advertising purposes.
First month’s fees, costume deposits and competition fees are not refundable for any reason.
Monthly fees are calculated on yearly session fee of a minimum of 32 weeks with a maximum of 36 weeks split evenly over 9 months. Season starts 2nd week of September and ends as of the recital in June.
Missed classes are non-refundable.

TERMINATION POLICIES
Anyone wishing to withdraw from enrolment of any classes at THRIVE DANCE ACADEMY must notify the studio IN WRITING and this notice must be received 30 days in advance or you will forfeit your monthly fee. Exceptions include health and medical reasons- a doctor’s note must accompany withdraw notice. NO TERMINATIONS WILL BE ACCEPTED OVER THE TELEPHONE AND MUST BE SUBMITTED IN WRITING OR VIA EMAIL- info@thrivedanceacademy.ca. Terminations from the program or dropped classes will NOT BE ACCEPTED AFTER DECEMBER 31 of the current dance season. (health or medical reasons excepted…doctor’s note required). Withdraw after this date and you will be responsible for submitting payment IN FULL for the remainder of the dance season. Anyone not abiding by this or putting stop payments on cheques will be subject to legal actions.
