Waiver of Liability / Acknowledgment of Risk

READ CAREFULLY

I understand and agree that in participating in any dance camp, dance class, workshop, rehearsal or performance, there is a possibility of physical injury and, in rare circumstances, death. I agree to release and hold Base Dance Performing Arts Academy including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Base Dance Performing Arts Academy liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I understand that I should be aware of my, and/or my child’s physical limitations and agree not to exceed them. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.

Membership, 30 days termination notice required

For regular classes, I agree to give 30 days’ notice via email or written communication to add, change or drop any classes including terminating my membership, the date of the communication will be taken as the date of notice for the period. I agree to pay the monthly or termly membership unless my membership is terminated without liability, suspended or transferred in accordance with these terms. The Academy reserves the right to change the day, time and location of the sessions. Should this be restrictive to attendance the minimum notice period will be waived.

(To be agreed by a parent for students under 16 years, or by the student themselves if over 16 years) I, being parent/guardian of the above named child* hereby give permission for an authorised BDPA representative to give the immediate necessary authority on my behalf for any medical or surgical treatment recommended by competent medical authorities, where it would be contrary to my/child/ward’s interest, in the doctor’s medical opinion, for any delay to be incurred by seeking my personal consent. NB. Please note that a young person can give their own consent for medical treatment if they are over 16 years old

Video and Photography

If you do not agree to our photo and video consent form, the member may not be able to participate in events and programs which photo and video consent is required to take part.

I hereby consent to Base-Dance LTD and Partners to using images of my child caught in video recordings, and/or photographs, I also consent to them being used for other marketing and publicity-related purposes and used in other publications and I understand that they may be published on the Base-Dance website or another website or elsewhere. I understand that:
•their images will be held in accordance with the Data Protection Act;
•that the images of my child captured in the video recordings and/or photographs will be the copyright of the Academy and any other intellectual property which arises in the recordings will also belong to the Academy;
•My child and I hereby agree to irrevocably assign all property rights in my child’s performance and/or recordings to the Academy;
•My child and I hereby agree to waive all moral rights in my performance in the film and/or recordings to the Academy.
•I can ask the Academy to stop using my child’s images at any time, in which case they will not be used in future publications but may continue to appear in publications already in circulation.
