Registration Information/Policies/Fees

Tuition is based on the entire season of dance. Our season runs September through June. Tuition is not based on the number of classes in a month or on attendance. There is NO refund for missed classes, ie., no pro-rated classes. If your dancer is not feeling well and if you test positive for covid, please contact us or email us - You can arrange to make up your class at our other studio location at a convenient time for you. Your dancers safety and health are our first priority.
Monthly tuition installment amount is based on the number of hours (classes) taken per week.¨ *Family discounts apply only to siblings under the same account.
I am aware that the first month of classes is offered at a reduced rate. There are only a half a month of classes in September. My tuition in the months of October - June will be double this amount. Family discount fees will be in effect:
2 students: 10% discount 3 students: 15% discount 4 students: 20% discount 5 or more... God Help you, we'll work something out!!

In registering your dancer, this means you agree to all information provided below. We encourage your questions. Thank you!

You must be over the age of 18 to register for an account. I, as a parent of a dancer, agree & acknowledge to all of the following:

REGISTRATION FEE:
Upon your decision to dance with O'Malley Irish Dance Academy, a Registration Fee will be charged, which will guarantee your spot in classes. Sibling discounts apply when registering more than one family member.

PAYMENTS:
Payments are DUE on the 1st day of each month for classes during that month. A LATE CHARGE of $15 will be charged after the 5th day of the month if there is a balance on the account. You are required to be on AUTO-PAY for Tuition and any owed balance - Any other fees may be paid for via Checks, Cash or Credit Cards (fee to use credit card) are all accepted forms of payment. Credit Card fees vary, depending on your chosen credit card company. Currently, associated fees through our merchant account are 2.9% fee. Returned check fees will be applied to your account- $30. By setting up your account on auto-pay via your Dance Studio Pro Parent Portal Account this enables you to be in control of your dancers account and enables us to communicate with you. You will be emailed a login/password upon registration. There you will have access to your Personal Parent/Dancer Account and may view all important items which are constantly updated via our Dance Studio Pro system.
Auto Pay is Strongly encouraged. Upon registration in the portal, your account will be charged a registration fee and you will be prompted to pay. If you do not pay this fee, your account with your dancer’s chosen classes will not be accepted in the system and your dancer will not be officially enrolled. Make sure to do this step!

EMAIL COMMUNICATION:
You must check your email for important updates for your dancer. This is very important and is our major form of communication. These updates contain vital and pertinent information that you and your dancer will need for events. It is your responsibility to check emails for your dancer. If you are not receiving emails from us, please let the office staff know immediately so we may correct any problems.


HOLD HARMLESS CLAUSE:
I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is an inherent risk of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of O'Malley Irish Dance Academy classes, rehearsals, performances or activities. I also exempt, release and indemnify O'Malley Irish Dance Academy, its owners, agents, volunteers, assistants, contractors, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by O'Malley Irish Dance Academy. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold O'Malley Irish Dance Academy, its owners, agents, volunteers, assistants, contractors, employees, guest artists, faculty members, and/or students liable for such damage, loss or injury. I understand that my child dancer/s and I should be aware of our physical limitations and agree not to exceed them. If I am signing this waiver for my child/ren, I certify that I am the parent or legal guardian and have the right to waive these rights.

Consent to Photographs / Social Media
Our school is involved in a great many activities during the course of the school year. Occasionally, we are photographed / videoed from various media sources including, but not limited to Newspapers, television and film during our performances. It is a good resource for our school to be seen in various forms of media and the children love nothing better than seeing themselves on TV or in the paper. Additionally, We have a FB page, a Twitter Account and an Instagram account for the school, which is managed by my staff and I. We will post pictures of our dancers at different events throughout the year. Finally, our end of the year show is always videotaped as a keepsake for the dancers. For the protection of all involved, we need your consent to this waiver in order for your child(ren) to participate.
If you do not wish to allow your consent please speak to me and , I will remove your child(ren) from any in-class pictures we take and they will not be a part of the show team as I cannot control what happens outside of my class. Thank you so much for your cooperation.
Permission is granted to O'Malley Irish Dance Academy to use photographs of students for publicity purposes.

Let's Stay Healthy!

The Studio:
ï‚· Our studio has been fully cleaned & sanitized
ï‚· Posters with proper sanitizing and staying healthy reminders have been posted.
ï‚· Hand Sanitizer Dispensers have been installed
ï‚· The studio / cubbies / common areas will be disinfected between each class.

Students:
ï‚· All students are expected to have THEIR OWN hand sanitizer with them and a water
bottle clearly marked with their name on it.
ï‚· All belongings are to be placed inside a closed dance bag at the beginning of class and will
have their own designated space to store it. They may not go and take out
phones/personal items during class.
ï‚· Students are required to wait to enter the studio until the previous class has been
dismissed. When applicable, please arrive at class with dance shoes ON! Teachers /
assistants will no longer be tying shoes for dancers.


Classes:

ï‚· All classes will be timed accordingly to allow distancing between the arrival / dismissal between classes.
ï‚· All payments must be electronic. Either Venmo, direct payment with a credit card, and Auto Pay .

Parents:
ï‚· Please check your child's dance bag prior to coming to class. Ensure they have their own hand sanitizer & water bottle.
ï‚· For children that cannot tie their own shoes, please have them arrive with shoes on.
ï‚· Please be sure children use the bathroom prior to arriving at class.
ï‚· If your child is sick, keep them home!
ï‚· If your child or anyone in your household is diagnosed with COVID19, follow NYS guidelines regarding testing and quarantining before returning to dance class.
ï‚· If you child runs a fever do not allow them to return to class until they are fever free, without the use of medication, for 72 hours.
ï‚· As much as we LOVE to see parents in the studio during classes, that is strictly prohibited now! Please email any questions to the school email:omalleyirishdanceacademy@gmail.com. It will be addressed and a response sent within 1 day.

As a member of O'Malley Irish Dance Academy, as a parent of the dancer/s, we agree to abide by the Registration Agreement including all Rules, Requirements, Tuition/Fees, Attendance and Dismissal Policies as stated in this agreement. Upon Submission of my registration, I am in agreement with all above terms and conditions.

Online Registrants agree to all of the above by accepting this waiver and submission of your chosen classes for registration.

Your parent portal contains a wealth of information for you and your dancer (Important Event Dates, Fees Due etc.- you can manage your account from the portal.

COVID-19 WAIVER FORM/ACCEPTANCE DECLARATION
WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19
ASSUMPTION OF RISK / WAIVER OF LIABILITY AGREEMENT

In consideration of being allowed to participate in lessons, competitions, performances, and other activities in conjunction
with O’MALLEY IRISH DANCE INC., the undersigned acknowledges, appreciates, and agrees that:

1. Travel to and from and participation in practices, rehearsals, group lessons, private lessons, performances, local,
regional, national and worldwide competitions and other related events and activities pose a risk of possible exposure to
and illness from infectious diseases including, but not limited to, MRSA, influenza, and COVID-19. Although rules,
precautions and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE
NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms/conditions for participation as regards protection
against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I
will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD
HARMLESS O’MALLEY IRISH DANCE ACADEMY INC. , its officers, officials, agents, and/or employees, other participants,
sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event
(“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property,
WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS,
UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY
WITHOUT ANY INDUCEMENT.

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the
provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal
responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my
child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and
agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree
to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or
participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent
provided by law.
