Liability Release Form:

I understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of
dance. In recognition of the acknowledgement risk of injury, I knowingly and voluntarily waive all rights and/
or causes of action of any kind, including all claims of negligence arising as a result of such activity from which
liability could accrue to Tippy Toes Dance Studio, Inc, its' members, officers, agents, employees, instructors,
assistants, subsidiaries and all affiliated entities (hereinafter collectively referred to as Tippy Toes Dance
Studio).

The noel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization.
COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a
result, federal, state and local governments and federal and state health agencies recommend social
distancing and have, in many locations, prohibited the congregation of groups of people.

Tippy Toes Dance Studio has put in place preventative measures to reduce the spread of COVID-19 and other
communicable diseases; however, Tippy Toes Dance Studio cannot guarantee that you or your child(ren)
will not become infected with COVID-19 or any other communicable disease. Further, attending
Tippy Toes Dance Studio could increase your risk and your child(ren)'s rist of contracting COVID-19 and any
other communicable disease.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and any other communicable
disease and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19
or any other communicable disease attending Tippy Toes Dance Studio and that such exposure or infection
may result in personal injury, illness, permanent disability or death. I understand that the risk of becoming
exposed to or infected by COVID-19 or any other communicable disease at Tippy Toes Dance Studio may
result from the actions, omissions, or negligence of myself and others, including, but not limited to, Tippy
Toes Dance Studio employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child
(ren) or myself (including, but not limited to, personal injury, disability and death), illness, damage, loss, claim
liability, or expense, or any kind, that I or my child(ren) may experience or incur in connection with my child
(ren)'s attendance at Tippy Toes Dance Studio or participation in Tippy Toes Dance Studio programming.
(Claims) On my behalf, and on behalf of my child (ren) , I hereby release, covenant not to sue, discharge, and
hold harmless Tippy Toes Dance Studio, its employees, agents, and representatives, of and from the Claims
including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.
I understand and agree that this release includes any Claims based on the actions, omissions, or neglegence of
Tippy Toes Dance Studio, its employees, agents, and representatives, whether a COVID-19 or any other
communicable disease infection occurs before, during, or after participation in any Tippy Toes Dance Studio
program.

I hereby agree to release Tippy Toes Dance Studio and hold Tippy Toes Dance Studio harmless of all liability,
and hereby acknowledge and I knowingly assume full responsibility for all risks of physical injury arising out of
active participation in dance on behalf of the participants.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing
assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange
for the privilege of participation.

I also give Tippy Toes Dance Studio permission to use my child's picture in or on any form of advertisement for
Tippy Toes Dance Studio or a Tippy Toes Dance Studio affiliated event.

If I am a minor, my parent and/or legal guardian has also signed this document releasing Tippy Toes Dance
Studio from any and all such liability described above and has acknowledged that I am knowingly and
voluntarily assuming all risks of injury inherent to this activity.

The participant has my permission to participate in Tippy Toes Dance Studio events and activities. I warrant
that the below information is complete and correct, I further release Tippy Toes Dance Studio of all
liabilities associated with my child's attendance at Tippy Toes Dance Studio.

______$30 non-refundable registration fee and signed rules must accompany registration form to have your space reserved. When registering between July 1-31 registration fee is $20.00.
______The instructor has the final say as to what class(es) your child is placed in.
______Tippy Toes Dance Studio does not carry medical insurance for its students. It is required that all dance students be covered by their own family insurance and if injury occurs, it is understood that the student’s policy is the only source of reimbursement.
______Leotards, tights and appropriate dance shoes are required for all classes. This includes no panties under your tights and leotard. Proper shoes in specific colors are required. Please label all shoes with your child’s name and do NOT wear them outside of the studio at any time.
_____****Black leotard and pink tights are required for all ballet classes Levels 1-4.***
______Please help class to run smoothly by going to the restroom before or after class. Class time is for learning not visiting the restroom.
______No gum, food, drinks, or CELL PHONES allowed in the studio by parents, dancers, siblings or friends.
______The only visible piercing allowed on our dancers is in the ear lobes of girls. If there are any other piercing, please have this removed prior to entering the studio. Absolutely no tattoos are allowed—NO exceptions.
______Students should not be dropped off earlier than 15 minutes prior to class or stay longer than 10 minutes afterwards. They should remain inside until their ride arrives.
______Please walk your student into the lobby or make sure they have gone inside before leaving.
______Disruptive behavior will not be tolerated. If this occurs repeatedly, parents will be contacted and if it continues, your child will be sent out of class.
______Please check your folder each week for flyers, bills and items left behind. The folder will be labeled with your child’s name.
______Tuition is due no later then the 10th of the month. There are no discounts or late fees.
______September through May will be charged full month’s tuition. Whether we have a short month (3 weeks) or long month (5 weeks), tuition remains the same.
______Please place your child’s name in the memo section of your check.
______You may pay in advance for fees; however, there is no discount.
______ Please put all payments in a TTDS envelop and place it in the payment box located in the lobby.
______There are no refunds for missed classes. These can be made up in an age appropriate class, even if it’s a different style than what the student is taking.
______There must be a 30-day written notice before your child quits their dance class. You are responsible for tuition up to the 30-day notice.
______Our recital will be held at Paramount Theater Goldsboro, NC. Tickets will be sold through the parent portal. Pricing will be set at a later time. Anyone that wants a SEAT must have a TICKET this includes students and children 2 years old and older.
______All classes are closed session. Please do not open the door during class time. When classes are over, the instructor will open the door.
______Holiday Breaks: Please refer to the Calendar of Events handout. We will have classes on teacher workdays and some student holidays (ex. Martin Luther King Day we will be having class).
______In case of inclement weather, there will be a message on the answering machine at the studio about any canceled classes and Facebook. You can also check our website tippytoesdancestudio.com for cancelations. Be sure to make up any missed classes by getting a schedule from the office.
______Class changes will not be allowed after December 31.
______To be able to participate in our spring recital your account must be up to date. No one will perform with an over due balance.
I have read the studio policies of Tippy Toes Dance Studio received a copy and agree to comply with them.
