The terms 'We' and 'Us' in this document refer to Lateral Business Solutions Australia Pvt Ltd trading as "Math Minds", the provider of services and the terms 'I', 'Me and 'You' refers to the client, the user of the services, and / or the person responsible for the payments for the use of services.
GENERAL CODE OF CONDUCT
My child hereby accepts a place with Math Minds and I certify that all details on the enrolment form or entered directly in the parent portal are correct.
I authorise the Math Minds Centre Director or Coach on duty, where it is impractical to communicate with me, to consent to such medical and/or surgical treatment that may be deemed necessary. I consent to my child being provided with my email identity, and all their creations related to their Math Minds work will belong to Math Minds and can be used for demonstration, training and promotion purposes. This may also include digital recordings of their image at the centre in a supervised environment.
I consent to my child using shared headsets and devices from the centre if they have not brought their own. I have read and understand the Math Minds Centre Policies below and undertake to abide by these. I have read and agree with the Payment Terms as explained to me at assessment and shown the policie below. I recognise that prices and policies are subject to change with adequate notice. If I do not object, in writing, within 4 weeks of the notice being emailed to me, the relevant change carries my agreement.
COVID19
All members are responsible for assisting us in creating and maintaining a safe and hygienic environment inside the learning centre by limiting the time and person to person contact at the centre. To that end, the following procedures and policies have been added to the general terms and conditions below:
1. Arrive on time, and bring only essential items to the centre. (NOTE: students will need to bring adequate water for the time they will be at the centre).
2. Bring your own laptop and headset. If you use one provided in the centre please sanitize it before and after use.
3. Maintain good hygiene practices while at the centre. Wash/sanitize hands before and after every class, before you arrive at the centre and after you leave. Maintain social distancing (1.5m) wherever possible within the centre.
4. Eat off-site, and no food or drink is brought into the centre. If exhibiting any signs of illness, please stay home. Staff reserve the right to refuse entry to any Members exhibiting any signs of illness.
5. If you, or any of your close contacts test positive, you will inform the centre director immediately.
6. No parents or guardians will be allowed inside the centre, all drop off and pick-up will need to occur outside the centre. The centre will have no more than 1 family at any one time, with prior appointment.
7. Follow all signs and instructions from staff.
Attendance
Regular attendance over a long period of time is critical for your child's success. Toilet breaks are not encouraged during the session, please come prepared. Emergencies are exceptions.
Makeup Sessions
All learning sessions must be scheduled in advance. Cancellations must be made 24 hours in advance on the parent portal for a free make-up session. This make-up session will expire if it is not requested and completed within 4 weeks of the class having been missed.
No refunds or credits will be given for missed classes.
Coaches
The Math Minds centre is staffed by carefully selected and trained coaches who are approved to work with children, by the Government of Victoria, or supervised by an approved personnel as required by the Government of Victoria. Students will benefit by working with all our coaches, and we do not honour requests for your child to work with a specific coach, unless it is a Private Session.
Centre Hours of Operation
The opening hours and days of operation for the centre may vary depending on the time of year. Math Minds is open on public holidays (unless advised otherwise) and missed classes must be requested and made-up within 4 weeks of the missed class.
Math Minds is open during school holidays and is not tied to school terms. We do not close for term breaks and close only for Christmas and New Year for about 4 weeks and parents are advised of these dates in advance.
Holiday Holds
Students are permitted to put their membership on hold for up to 8 weeks in a 12 month period, for 3 weeks or more at a time. To place your child's membership on hold you must inform the Centre, in writing, at least 2 weeks prior to commencement of the holiday hold. Your request for Holiday Hold is conditional on your account being in good standing and we will confirm that you will not be billed for the time that you put the account on hold. Individual weeks or days may not be put on hold. No credit or make up is provided for delayed notification or missed sessions. We try to maintain your time slot while on Holiday Hold, however this is not always possible and on your return, you may find yourself in a different time slot.
Billing, Cancellation and End of Membership
Math Minds is a fair, reasonable and responsible member of society and inspires our students to be the same. New students will benefit from at least a 3 month association with the Math Minds Learning approach. At the time of registration you are required to pay the annual registration/resource fee. You can cancel your membership at any time by giving us 4 weeks paid notice.
If your account has two or more failed payments you are required to provide a 4 week withdrawal notice deposit.
Your 4 weekly tuition fee is required to be paid in advance. If you wish to cancel your student's membership, please do so in writing at least 4 weeks prior to the intended last session. Please note the 4 week paid withdrawal notice cannot run concurrent with a holiday hold, and will require 4 weeks payment at the end of the holiday hold period.
A late fee upto 10% will be assessed for any fees not paid before the commencement of the billing cycle. Every parent is required to set-up an auto-pay feature to minimise this fee. All annual and monthly fees, disclosed at time of registration, are paid in advance. A dishonour fee will apply for any attempted payments due that are unsuccessful for any reason.
Please promptly update your credit card or billing account information in your portal if your account number or card details change.
Students may only attend sessions if your account is in good standing.
Finally
This agreement is valid from the time it is digitally date stamped as having been accepted by me and is valid for any services I may use from Math Minds now and in the future.
