Permission and Assumption of Risk
I/we give our permission for the above named student(s) to participate in tumbling, realizing that there are risks of physical injury to the participant(s). Considering all personal risks, on behalf of myself and the participant(s), I voluntarily waive, release, discharge, and hold harmless Hawli Landvatter, Tumble Shack, LLC, its employees, instructors, supervisors, subcontractors, and volunteers from all claims, including those based on negligence, for all injuries to participant(s), no matter how severe.
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that The Tumble Shack LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that The Tumble a Shack LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other clients and their families.
I voluntarily seek services provided by The Tumbleshack LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold The Tumble Shack LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from The Tumbkr Shack LLC. I understand that this release discharges The Tumbke s Shack LLC from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from The Tumble Shack Studios LLC. This liability waiver and release extends to the studio together with all owners, partners, and employees.
At registration, we require payment for May 2021 tuition (this is called a "class registration fee" but it is your May tuition), a $25 registration fee per tumbler, or $50 per family. This is a non-refundable deposit. Please pay your registration fee and May tuition through our parent portal or by cash.
Monthly tuition is due the first of every month. Tuition is based on our yearly season program split into 10 monthly payments. The price is the same each month whether it has 2, 3, 4, or 5 classes. It will all even out throughout the season.
If you choose to have your card automatically billed each month there will be a 3% convenience fee added to your tuition. Another alternative is to set up your online payment using a checking routing number and it is only charged a .75% convenience fee, or you can pay by cash and avoid the convenience fees.
Monthly tuition for the 2020-2021 tumbling season will be:
$50 a month for 45 min preschool classes
$55 a month for a 1 hour class once a week
$60 a month for a 1 hour 15 min. class once a week
$65 a month for a 1 hour 30 min. class once a week
$100 a month for 1 hour class twice a week
$110 a month for a 1 hour 15 min. class twice a week
$120 a month for a 1 hour 30 min. class twice a week

Pick up and Arrival: Please be prompt at drop off/pick up. Kids should not go outside of the gym until their ride has arrived.
No lobby: To maximize our tumbling space we do not offer a lobby space for parents to sit and wait. Parents and siblings are welcome to watch from outside.
Missed Classes: Make up classes are available for missed classes. You will be allowed only one make up class a month. I do not pro-rate. Payment will not be refunded or transferred to the next month so make sure to schedule any makeup classes. Monday classes will be aloud 1 scheduled makeup during the season to make up for Monday holidays. These must be scheduled prior the class time.
Class Schedule: We will follow the Gilbert Public School schedule. If there is NO school, there is NO class unless otherwise instructed.
Students who wish to withdraw MUST give at least 30 days notice.
