I am a parent/guardian wishing to enroll my child in Dimensions In Dance.

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Dimensions In Dance II, Ltd and Dimensions On Stage, Inc. classes, camps or events and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Dimensions In Dance II Ltd and Dimensions On Stage, Inc. may result from the actions, omissions, or negligence of myself and others, including, but not limited to volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at Dimensions In Dance II, Ltd and Dimensions On Stage, Inc. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Dimensions In Dance II, Ltd and Dimensions On Stage, Inc., its employees, volunteers, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Dimensions In Dance II, Ltd and Dimensions On Stage, Inc. event.

Students (and parents/guardians who are enrolling minors) assume a certain amount of risk associated with dance, tumbling, and other fitness related activities. I agree to waive the right to any legal action for an injury sustained on studio property or during online class offerings prior to, during, or after class or event participation at Dimensions In Dance II, Ltd and/or Dimensions On Stage, Inc.

I understand that Dimensions In Dance requires a dress code for its classes and I agree to ensure my child is dressed safely and appropriately for dance class. This means no jewelry, jeans, oversized clothing or anything the teacher deems inappropriate for the style of dance. Correct footwear is a requirement for class and a student may be asked to sit out for their own safety if they are not wearing appropriate dance attire.

Cell phones, valuables, food and toys are not allowed in class. Please silence cell phones before entering the dance room. Dimensions In Dance is not responsible for any valuables left behind.

By registering, I understand that my child’s photo and/or video may appear on Dimensions social media, website, newsletters for the purpose of marketing and demonstrating our services. My child’s last name will never be used without my direct consent.

Tuition each month is due by the 10th day of every month. You may pay in cash or check at the studio up until the 10th day, thereafter your credit card will be charged your full tuition price plus a 3% convenience fee. A payment method must be kept on record for every student’s account.

Tuition is due monthly regardless of attendance. If you choose not to come to class for any reason, you are still responsible for payment in full. We do not pro-rate classes for absences.

Withdrawals from class must be emailed to the studio at DimensionsInDanceNY@gmail.com in order to not be charged the monthly fee.

Costume Deposits are due in November. We must receive a costume deposit in order to purchase a costume for the Recital. Once costume orders are placed, this deposit is non-refundable.

Costume Balances/Recital Fees are due in March. All costume balances/recital fees must be paid in full in order to receive a costume for your child’s performance.
