We at All That Dance understand our obligation to make sure that all of our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this waiver, you are hereby releasing All That Dance and all of its employees from all claims on account of any injury which may be sustained by your child while attending any dance class or event associated with All That Dance. In signing this waiver, you also acknowledge your responsibility in paying registration, tuition, costume, and or recital fees on time. You also affirm that you carry proper medical, health and hospitalization insurance which you consider adequate protection for your child and All That Dance.
All That Dance Studio Policies 2022
______I understand that I must pay a registration fee for my dancer of $20.00 per student and additional family members are $10 per child.
______I understand that this fee is required to hold your spot in class.
______I understand that this fee is non-refundable.
______I understand that TUITION IS DUE ON OR BEFORE THE FIRST DAY OF CLASS.
______I understand that tuition is charged per class per student and students who take more than one class are eligible for the second, class discount.
______I understand that tuition must be paid on time, in order for the child to participate in dance class and if tuition is not paid on time that my child will be removed from dance class.
______I understand tuition that is not paid on time will automatically be charged a $15.00 late fee by the 10th of the month.
______I understand that tuition fees are NON-REFUNDABLE.
______I understand that a returned check fee is $25.00. I understand that if my check is returned, that I must pay the fees for insufficient funds. Then I can only pay with cash, cashier’s check, credit card or money order. I understand that monthly tuition must have a card on file (no exceptions).
______I understand that I must give notice of my child dropping a class by the 10th of the month or I will be responsible for paying the entire month’s tuition.
______I understand that I must keep my credit card up to date online.
______I understand that attendance is recorded at the beginning of each class.
______I understand that I must plan to arrive 5-10 minutes before class is scheduled to start and wait outside until my teacher brings me into the studio.
______I understand that I should call the studio in advance of my child’s absence and that make-ups for such absences must be scheduled with my child’s teacher or the Studio Owner.
______I understand that if class is cancelled by the teacher, that the teacher will be responsible for making up the class.
______I understand that parents are NOT allowed inside the studio to observe classes.
______I understand that only the parent may observe class from outside the studio via the Zoom app.
______I understand that only dancers and teachers are allowed inside the building.
______I understand that my child must dress out appropriately for dance class.
______I understand that my child must arrive dressed and ready to dance.
______I understand that hair must be pulled back from their face; in a bun, ponytail or French braid.
______I understand that there will be no sharing of hair products, brushes or bobby pins.
Ballet: Pink tights, black leotard, pink, leather ballet shoes. Hair must be in a bun.
Jazz: Black jazz pants, shorts or capris, tan jazz shoes, leotard or camisole.
Tap: Black tap shoes, any solid color leotard and warm-up pants.
Hip Hop: Black hip hop sneakers and loose, comfortable clothing.
Lyrical: Same as jazz, foot undies required.
Acrobatics: Leotard and shorts, no shoes. Hair in a low ponytail.
_____I understand that if my child is not dressed out appropriately for class, that they may not be able to participate in dance at the teacher’s discretion.
Spring Holiday-Spring Break (March 14-18, 2022)
______I understand that all other holidays/breaks are not included in tuition and therefore will not be made up. ______I understand that any snow days or other events and/or closures due to Covid-19, will be made up online via the Zoom app instead of in-person.
_______Class Descriptions: Dancers must meet the requirements as listed online for each class.
______I understand that students must be able to perform the required skills in order to participate and will be placed only by teacher recommendations and are subject to the approval of the Studio Owner/Director.
______I understand that dancers younger than four years old, must be potty trained, willing to go into the studio without their parent and that I will need to stay nearby, in the parking lot in case of bathroom emergency.
_______Recital: I understand that performing in the recital is optional.
______I understand that if I do choose for my child to participate in the show(s), that there are extra costs for costuming, rehearsals, tickets and pictures.
______I understand that all fees must be paid in full and on time.
______I understand that my child must attend all rehearsals for the recital as scheduled in person in order to perform in the show.
______I understand that there are NO refunds for costumes, recital, tuition fees and any extra fees for the recital (tickets, photos, videos).
_______Misc: I understand that I must notify the studio if someone else is picking my child up after class.
______I understand that teachers, who must wait for my child to be picked up 15 minutes or more after class, will be charge a $10/per minute sitting fee.
______I understand that my child should not show up for dance class until 15 minutes prior to the class start time.
______I understand that in case of an emergency, I should contact the studio office as soon as possible
______I understand that my child may not leave the studio with another dancer, another parent or person unless the studio has prior, written permission from the parent of the dancer.

______ABSOLUTELY NO ONE IS ALLOWED INSIDE THE DANCE STUDIO, EXCEPT TEACHERS & DANCERS
______NO FOOD IS ALLOWED INSIDE THE DANCE ROOMS/STUDIOS.
______ALL DANCERS MUST BE DROPPED OFF AND PICKED UP PROMTLY AT THE FRONT DOOR.
______DANCERS MUST BRING A WATER BOTTLE TO CLASS. PLEASE MARK NAMES ON ALL ITEMS.
______ MASKS ARE OPTIONAL INSIDE THE STUDIO AND WHILE DANCING.
______CLASS SIZE IS LIMITED, AS WELL AS OCCUPANCY IN THE BUILDING
______DANCERS WILL REMAIN 6 FEET APART AT ALL TIMES.
______NO PROPS, NO PARTNERING OR PHYSICAL CONTACT BETWEEN DANCERS.
______WE PREFER ALL PAYMENTS TO BE PAID ONLINE. PARENTS CAN DROP OFF CASH OR
CHECK IN THE STUDIO OFFICE. WE WILL NO LONGER USE SQUARE FOR TUITION PAYMENTS.
______PRESCHOOL AND KINDER AGE DANCERS MUST HAVE A PARENT WAITING IN THEIR CAR.
______DANCERS WHO ARE SICK, RUNNING A FEVER, OR HAVE A FAMILY MEMBER WHO IS SICK,
MUST TAKE CLASS ONLINE. FOR THOSE WHO MAY HAVE BEEN EXPOSED TO SOMEONE WHO IS POSITIVE WITH COVID-19, THE DANCER MUST TAKE CLASS ONLINE AND NOTIFY THE STUDIO IMMEDIATELY.
I HEARBY WAIVE, RELEASE AND DISCHARGE ALL THAT DANCE, INC. and all divisions thereof any all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and or instruction of dance classes, camps, intensives, private instruction, choreography or any activity I may participate in.
