Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. has put in place preventative measures to reduce the spread of COVID-19; however, M.A.D.E. STARS cannot guarantee that you will not become infected with COVID-19. Further, participation could increase your risk of contracting COVID-19.

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by participation; and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at M.A.D.E. STARS may result from the actions, omissions, or negligence of myself and others, including, but not limited to, M.A.D.E. STARS's employees, volunteers, and program participants and their families

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my participation at M.A.D.E. STARS. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless M.A.D.E. STARS, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of M.A.D.E. STARS, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation at M.A.D.E. STARS.

I represent that I have adequate insurance to cover any injury or illness I may suffer or cause while participating in this activity, or else I agree to bear the costs of such injury or illness myself. I further represent that I have no medical or physical condition which could interfere with my safety in this activity, or else I am willing to assume – and bear the costs of – all risks that may be created, directly or indirectly, by any such condition.

I agree that if I am exposed or infected by COVID-19 during my participation in this activity, then I may be found by a court of law to have waived my right to maintain a lawsuit against the parties being released on the basis of any claim for negligence.

I have had sufficient time to read this entire document and, should I choose to do so, consult with legal counsel prior to signing. Also, I understand that this activity might not be made available to me or that the cost to engage in this activity would be significantly greater if I were to choose not to sign this release, and agree that the opportunity to participate at the stated cost in return for the execution of this release is a reasonable bargain. I have read and understood this document and I agree to be bound by its terms.

I agree that I will practice safe social distancing and clean hygiene during my participation at M.A.D.E. STARS.

Covid-19 Screenings & Symptoms



1. In the event that my child presents any of the below symptoms of Covid-19, I will not drop them off to M.A.D.E. STARS Instructions.
2. In the event that my child has a temperature higher than 100.2 F at check-in, or presents in any of the symptoms below, I will abide by staff orders, not allowing my child to enter the premises.
3. In the event that my child possesses any of the below symptoms or have a temperature higher than 100.2 during the dance class hours. I will immediately pick them up, within a 30-minute time frame from the staff contact.



Covid -19 Symptoms
• Cough
• Shortness of breath or difficulty breathing
• Or at least two of these symptoms
• Fever
• Chills
• Repeated shaking with chills
• Muscle pain
• Headache
• Sore throat
• New loss of taste or smell I agree that if my child possess any of the above symptoms, I will abide by the above rules & regulations of M.A.D.E. STARS Performing Arts School. If for any reason, I do not abide by the Covid-19 rules and regulations of M.A.D.E. STARS Performing Arts School, I will be dismissed from the program, and still responsible for my monthly financial obligation.

2021-2022 Dance Contract



A. Registration

There is a $60 non-refundable membership fee due at the time of registration for new and returning students or $85 per family(includes leotard for combo classes and M.A.D.E. STARS t-shirt for HIP HOP only registrations.

B.Contract

This is a 10 month contract. Starting September 11th 2021 - June 18th 2022

C. Dance Class Placement

Placement at M.A.D.E. STARS Performing Arts School is based on the following criteria: grade level, technique, potential, commitment level, enthusiasm, attitude and behavior in class, attendance and stage presence. Students are placed in one of the 4 levels based on grade level and ability. We place students together in groups that we feel will work the best together in rehearsal and performance. All student placements are based on Instructor/Director approval.

D. Attendance & Time Commitment

Regular attendance in all dance classes is expected of at M.A.D.E. Stars Performing Arts School. Please note, regardless of missing classes, tuition is still your obligation.

PICK-UP
There will be a $1 fee every 1 minute a parent is late to pick up student, after the student’s last class of the day. Payment will be added to monthly bill. We do give parents a 10-minute grace period.

E. Tuition/Costume Fee/Recital Fee/Late Fee/Drop Fee

Costume Fee/Tight Fee - Fees are due in January 2022 & March 2022. Costume Fees are $80 per class/costumes.
Recital Fee- Recital Fees are due in January 2022 & May 2022. Recital Fees are $200 per family. Parents will receive 10 tickets with a face value of $20 for each dance recital.

M.A.D.E. Stars Performing Arts School 2021-2022 Dance Year tuition is paid monthly depending on the number of classes taken. Parents are able to make their payments between 1st-10th via Cash App, Venmo, or Parent Portal. Parents are responsible for all tuition fees & payments starting September 11th, 2021- June 18th, 2022 regardless of absences, dismissal, or leaving the program.

A $40 late fee will be charged to all accounts on the 11th. Students are not permitted to take classes or perform, until their balance is current, and the late fee is paid.

If there is an issue with the account, meet with management as soon as possible, as the account will continue to accumulate tuition charges, late fees, and drop charges, until legal action is taken. Due to the large amount of students enrolled in our program, and our COVID-19 class size limitations we are not able to take “special request dates” for payments– NO EXCEPTIONS!


F. Delinquent Account/Unpaid Balance

In order for your child to participate in weekly classes and activities, your account must be up to date, at all times. Delinquent accounts will result in students not being able to participate in dance classes, and turned away at check-in.

G. Teachers

*SUBSTITUTE TEACHERS: Qualified substitute teachers are used when M.A.D.E. STARS Instructors aren’t available. Substitute teachers consist mainly of M.A.D.E. STARS instructors, although “outside” Instructors from other professional studios may be called if necessary. Please note that parents and students are NOT notified in advance that a substitute teacher will be instructing a class. These substitute teachers will be vetted to ensure the safety and health of our students.

H. Parent Responsibility

It is the responsibility of the parent/guardian to be aware of all dance activities. The school will post all such notices on the Facebook Group Board “M.A.D.E. STARS Parent Page” ONLY. It is also the responsibility of the adults or parents to inform the school of any home address, email address, or telephone number change. It is the responsibility of the student/parent to notify M.A.D.E. STARS of any illness, vacation, financial problems, change in schedule or any other factors interfering with the established and hereby agreed upon M.A.D.E. STARS policies.

I. Closed Classes/Rehearsals

Due to the current severity of Covid-19 parents are not allowed at M.A.D.E. STARS during class hours.

J. COVID-19 PROCEDURES/CLOSING

1. If in any case a student or staff member is infected with Covid-19, all students who came in contact with that individual will be notified within 24 hours. That individual will remain anonymous for their privacy. The group of students & staff who came in contact will remain out of the studio for 14 days in quarantine. M.A.D.E. STARS will immediately set up Virtual Classes for those students for their 14 days, out of the studio.
2. In the event that M.A.D.E. STARS must close, due to Covid 19. All classes will be moved to a virtual platform. Students will remain in their same classes, with the same hours of dance. Parents are STILL responsible for monthly tuition.

3. At this current time we will be having small recitals by levels. If at any reason, M.A.D.E. STARS does not have a dance recital in-person or virtually due to Covid-19, M.A.D.E. STARS will waive the recital and costume fees.
