GENERAL LIABILITY AND TEACHER SPOTTING WAIVER FORM
I/we realize that participation in dance classes and activities could involve some possible personal injury.
Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and
parent/guardian) assume all risks related to the use of any and all spaces used by Ireland Academy of
Dance & Performing Arts. This is to include Virtual classes hosted at home or an out of studio setting.
I/we agree to release and hold harmless Ireland Academy of Dance & Performing Arts, including its
teachers, dancers, staff members and facilities used by Ireland Academy of Dance & Performance Arts
from any cause of action, claims, or demands now and in the future. I/we will not hold Ireland Academy
of Dance & Performance Arts liable for any personal injury or any personal property damage, which may
occur on or off the premises before, during or after classes. This is including virtual class instruction as
well. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our
behaviour in addition to any damage I/we may cause to the facilities utilized by Ireland Academy of
Dance.
I/we agree to allow Ireland Academy of Dance the permission to use photos and videos of the dancer(s)
for solely the promotion and marketing of the Ireland Academy of Dance.
I understand that Ireland Academy of Dance & Performance Arts is licensed, accredited, and insured. In
the event that I/we should observe any unsafe conduct or conditions before, during or after my/our
classes, I/we agree to report the unsafe conduct or conditions to the Executive Director, Artistic Director,
Individual Instructor or staff member as soon as possible.
TEACHER TECHNIQUE ADJUSTMENTS / SPOTTING
I/we realize that the participation in dance/acro classes and activities could involve physical
contact for spotting purposes and technical adjustments . By signing this release form, I/we (the
dancer and parent/guardian) assume all risks related to the use of spotting techniques and
technique adjustments by instructors, including the contraction of COVID-19. Our safe dance plan
policy will be in place.
I/we agree to release and hold harmless Ireland Academy of Dance & Performing Arts, including its
teachers, dancers, staff members and facilities used by Ireland Academy of Dance & Performance
Arts from any cause of action, claims, or demands now and in the future. I/we will not hold Ireland
Academy of Dance & Performance Arts liable for any personal injury, illness, or any personal
property damage, which may occur on the premises before, during or after classes. Furthermore,
I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in
